不同方案治疗流产后宫腔残留的疗效观察

摘　　要：目的：比较不同方案在治疗流产后宫腔残留的临床疗效。方法：本院2014年1月~2015年10月收治的157例不全流产患者随机分为清宫组52例、米非司酮组54例、妈富隆组51例,观察3组不同治疗方案阴道流血时间、血hCG值下降、月经恢复周期及月经干净后子宫内膜厚度,评估疗效及并发症的发生。结果：米非司酮和妈富隆组阴道流血时间少于清宫组（P〈0.05）,血hCG值下降至正常天数3组之间比较无差异（P〉0.05）,月经复潮天数妈富隆组低于米非司酮组和清宫组（P〈0.01）,月经干净后子宫内膜厚度妈富隆组高于米非司酮组和清宫组（P〈0.05）,3组疗效比较无差异（P〉0.05）,清宫组并发症高于米非司酮组和妈富隆组（P〈0.05）。结论：对于宫内残留物较小的不全流产患者药物保守治疗疗效满意,避免了清宫,减少并发症的发生,妈富隆能更快的建立正常的月经周期。Objective： To compare the clinical efficacy of different methods in treating incomplete abortion. Methods The clinical data of 157 patierits with incomplete abortion were collected in our hospital during January 2014 to October 2014, and divided into three groups： curettage group had 52 cases, mifepristone group had 54 cases, marvelon group had 51 cases. To observe the duration of vaginal bleeding, the decline of HCG, the time of menstruation restore and the thickness of endometrium after menstruation. To explore the curative effect and complication of three groups. Re- sults： The duration of vaginal bleeding in mifepristone group and marvelon group was shorter than that in curettage group, and the differences were statistically significant（P〈0.05）. There was no significant differences in the decline of HCG among the three groups（P〉0.05）. The time of menstruation restore in marvelon group was shorter than that in mifepristone group and curettage group, and the differences were extremely significant difference（P 〈0.01）. The thickness of endometrium after menstruation in marvelon group was thicker than that in mifepristone group and curet- tage group, and thedifferences were statistically significant（P〈0.05）. There was no significant differences in the cura- tive effect among the three groups（P〉0.05） ,and the incidence of complication in curettage group was higher than that in marvelon group and mifepristone group, and the differences were statistically significant（P〈0.05）. Conclusion The medicine conservative treatment is effective in treatment of patients with little decidual residue due to incomplete abor- tion, which had the satisfying curative effect , could avoid uterine curettage and decrease the incidence of complication, marvelon could adjust menstrual cycle quickly, it was worth promoting.
