针刺疗法治疗指屈肌腱腱鞘炎的临床研究进展

摘　　要：目的分析总结近35年来有关针刺疗法治疗指屈肌腱腱鞘炎的临床研究文献,明确针刺疗法治疗指屈肌腱腱鞘炎的临床研究进展。方法分别以针刺、针刺治疗及指屈肌腱腱鞘炎、弹响指、扳机指为检索词,1981—2015年为检索条件,对中国知识资源总库——CNKI系列数据库及Pub Med进行检索,并对检索结果进行分析整理。结果共筛选出30篇相关文献,发现研究大多数为病例报道,其余文献则均为单纯的临床对照试验(CCT),有系统的随机对照及设置随访之试验设计(RCT)较少。在研究方法上,多为疗效观察研究,但无统一的诊断与疗效评定标准,有关指屈肌腱腱鞘炎的针刺机理研究报导尚不多见。结论针刺疗法治疗指屈肌腱腱鞘炎具有操作简单方便,不良作用小的优势,临床疗效满意,今后应进一步完善针刺疗法治疗指屈肌腱腱鞘炎的临床研究实验设计,以阐明针刺疗法治疗指屈肌腱腱鞘炎的作用机理,使之更好地应用于临床。Objective To analyze the clinical research progress of acupuncture therapy in treating flexor tendon tenosynovitis in recent 35 years. Methods With acupuncture, acupuncture therapy, flexor tendon tenosynovitis and trigger finger as the search terms, from 1981 to 2015 as a search condition, China National Knowledge Infrastructure （CNKI） and PubMed was retrieved, and the retrieval results were collected and analyzed. Results It is found that most of the literatures were case reports, and the rest only related to the clinical control test （CCT） while systematic randomized control trial （RCT） with the follow-up visit were rarely involved in all 30 articles. For the researching strategy, most of the literatures were demonstrating on the effect of acupuncture treatment on flexor tendon tenosynovitis hut less on the treating mechanism. Meanwhile, the diagnostic criteria and curative effect assessment were not aligned. Conclusion The review found promising evidence for the use of acupuncture in the treatment of flexor tendon tenosynovifis on improving the related symptoms and restoring the function affected fingers. However, the results were limited by methodological flaws. To confirm the therapeutic effect of various kinds of acupuncture therapy, the experimental design should be improved for the clinical research on the therapies, so that the therapeutic effect and treating mechanism of the therapies can be further testified and better applied to the acupuncture practice on flexor tendon tenosynovitis.
