髌骨软骨退变分级对保留髌骨型全膝关节置换术疗效的影响

摘　　要：目的探讨髌骨软骨退变分级对保留髌骨型全膝关节置换术疗效的影响。方法选择132例膝关节骨关节炎患者为研究对象,按照Outerbridge分级分为I、II、III、IV级(四组)。均行全膝关节置换术(TKA),比较患者间术前,术后膝前痛情况、KSS评分、髌骨评分情况。结果四组患者膝前痛程度较术前均减轻,四组患者间膝前痛情况比较差异无统计学意义(>0.05)。四组患者术后KSS功能评分均较术前明显改善,四组患者间KSS功能评分比较差异无统计学意义(>0.05)。四组患者髌骨功能评分均较术前明显改善,四组患者间髌骨功能评分比较差异无统计学意义(>0.05)。结论不同髌骨软骨退变分级对保留髌骨型TKA患者膝前痛的发生及关节功能无明显影响。To study influence of patella cartilage degeneration classification on total knee replacement with retaining patellar model. 132 cases of osteoarthritis patients were selected as the research object, and they were divided into class I, class II, class III, class IV according to the Outerbridge classification. Anterior knee pain level and KSS score, patella score of patients between the four groups I, II, III and IV level were compared. Anterior knee pain of patients in I, II, III, IV level between the four groups were lighter than before, and differences of anterior knee pain condition of patients in four groups with were of no statistical significance（ P〈0.05）. KSS functional score in patella cartilage degeneration I, II, III, IV level of patients between four groups were obviously improved compared with preoperative, and KSS function score comparison differences between patients of four groups had no statistical significance（ P〈0.05）. Patellar function score in patella cartilage degeneration I, II, III, IV level of patients between four groups were obviously improved compared with preoperative, and patellar function score comparison differences between patients of four groups had no statistical significance（ P〈0.05）. Different patella cartilage degeneration classification type has no obvious effect on anterior knee pain and curative effect of retaining patellar TKA patients.
