摘　　要：目的:评价"糖卫士"APP应用于糖尿病延续护理的效果。方法:将项目小组自主研发的"糖卫士"APP与医院"糖尿病管理平台"对接后试用于临床。筛选符合纳入标准的患者共60人,分为实验组30人,对照组30人。对照组采用电话随访,实验组采用"糖卫士"APP进行延续护理。结果:干预3个月及6个月后,实验组患者的血糖和血脂等代谢指标、监测依从性均优于对照组(P<0.05),按时复诊率和满意度也好于对照组。结论:基于"糖卫士"APP对糖尿病患者进行延续护理,有利于改善其代谢指标、监测依从性、按时复诊率和满意度。Objective： To evaluate the effect of the ＂sugar Guard＂ APP applied in the transitional care of diabetes. Methods： The APP was applied to clinic after docking with diabetes management platform developed by the First Afifliated Hospital of Guangxi Medical University. A total of 60 patients who met the inclusion criteria were recruited and divided into experimental group and control group with 30 patients in each group. On the basis of the existing health education model, the control group used telephone follow-up, while the experimental group used the ＂sugar Guard＂. Results： After 3 and 6 months＇ intervention, The improvement of blood glucose,blood lipid and other metabolic parameters, monitoring compliance, timely referral rates and patient satisfaction in the experimental group patients were better than those in the control group （P〈0.05）. Conclusion： Based on APP diabetes transitional care model, we could manage the patients scientiifcally and improve the patients＇ metabolic indicators, monitoring compliance, timely referral rate and satisfaction.
