摘 要:目的 :探讨256层CT冠状动静脉同步双重成像的可行性及诊治价值。方法 :71例患者在256层CT冠状动脉扫描的基础上,增加延迟扫描时间、补充对比剂总量后进行心脏CT同步扫描。评价冠状动、静脉双重CT图像质量,分析相关解剖学结构。结果 :冠状动、静脉图像质量评分分别为(4.57±0.68)分、(2.93±0.26)分,合格节段分别为99.2%、100%;动、静脉主要分支开口、走行及其两者间关系显示清晰,测得了冠状静脉窦(CS)、心中静脉(MCV)、左室后静脉(PVLV)等开口大小、间距及MCV、PVLV与CS间夹角等数据。检出冠状动脉心肌桥55段、斑块110段、狭窄(≥50%)9段。结论 :256层CT心脏同步扫描能满意地显示冠状动、静脉双重血管,有助于无创评价冠状循环、发现心绞痛病因、制定心脏介入治疗方案。Purpose: To investigate the value of 256-slice CT synchronous dual imaging for coronary artery and coronary vein in the diagnosis and interventional therapy of cardiac diseases. Methods: Seventy-one patients underwent CT coronary angiography with prolonged delayed time and additional amount of contrast agent. The relationship of image quality and the anatomic clarity was assessed. Results: The quality grade of the coronary artery and coronary vein images was 4.57±0.68 and 2.93±0.26 respectively. The qualified rate of the image of the segment of coronary artery and coronary vein was 99.20% and100% respectively. The distribution of main branches and the relationship between vessels of coronary artery and vein were shown clearly in the CT image. The diameters of the orifices and the distance between coronary sinus(CS),middle cardiac vein(MCV) and posterior vein of left ventricle(PVLV) were measured. The angle between MCV/PVLV and CS were also obtained. In cases with coronary disease,myocardial bridges were detected in 55 segments,plaques in 110 segments and stenosis in 9 segments(≥50%). Conclusion: Coronary artery and vein can be clearly displayed by 256-slice CT synchronous dual imaging. This method has significance in providing a noninvasive method to evaluate the situation of coronary circulation,etiological diagnosis of angina pectoris,and to guide interventional electrophysiological treatment in cardiac disease.
