血府逐瘀汤对肋骨骨折伴血胸患者凝血功能及疗效的影响

摘　　要：目的:探讨血府逐瘀汤对肋骨骨折伴血胸患者凝血功能的影响及临床疗效。方法:128例肋骨骨折伴血胸患者以数字表法随机分为观察组(n=64)与对照组(n=64),对照组给予常规治疗,观察组再对照组基础上加用血府逐瘀汤治疗,对比两组患者治疗前后凝血功能变化及临床疗效。结果:治疗后两组患者PT、APTT、TT均明显增高(P<0.01),FIB、D-D均明显降低(P<0.01),且治疗后观察组TT显著高于对照组,FIB、D-D显著低于对照组(P<0.01);观察组有效率为100.0%(64/64),显著高于对照组的93.8%(60/64)(P<0.05);观察组住院时间显著短于对照组(P<0.01)。结论:血府逐瘀汤能够有效延长肋骨骨折伴血胸患者的凝血时间,降低血浆纤维蛋白原以及D-二聚体水平,提高治疗有效率,缩短总住院时间,值得推广应用。Objective： To discuss the influence on coagulation state and curative effect of Xuefu Zhuyu Decoction in patients with rib fracture combined with hemathorax. Methods： A total of 128 patients with rib fracture combined with hemathorax were randomly divided into the observation group（ n = 64） and the control group（ n = 64）. The control group were treated with conventional therapy and the observation group with Xuefu Zhuyu Decoction based on the control group. And then the coagulation function and curative effect were contrasted between the two groups. Results： PT,APTT and TT in the two groups were significantly higher and FIB and D- D significantly lower than before（ P〈0. 01） and TT in the observation group significantly higher than the control group＇s and FIB and D- D significantly lower than the control group＇s（ P〈0. 01）. The effective rate of the observation group was100. 0%（ 64/64）,significantly higher than 93. 8%（ 60/64） in the control group（ P〈0. 05）. Hospitalization time in the observation group was significantly shorter than the control group＇ s（ P〈0. 01）. Conclusion： Xuefu Zhuyu Decoction can effectively prolong the hemothorax,decrease the levels of plasma fibrinogen and D- dimer,improve the treatment efficiency and shorten the hospitalization time in patients with rib fracture combined with hemathorax. It is worthy of promotion.
