微小乳腺癌的超声恶性征象研究

摘　　要：目的探讨微小乳腺癌的超声表现,总结微小乳腺癌典型恶性征象。方法回顾分析105例经病理证实的微小乳腺癌(直径≤1 cm)患者(微小乳腺癌组)的超声资料,同时选择同期病理证实的较大乳腺癌(>1 cm)患者100例作为对照组(较大乳腺癌组),比较两组间的声像图差异和误诊率。结果较大乳腺癌组中的声像图恶性征象发生率显著高于微小乳腺癌组,形态不规则、纵横比>1、微钙化、内部血流情况、有无腋窝淋巴结转移两组间比较差异均有统计学意义(P<0.05),其中纵横比>1在微小乳腺癌组中发生率(34.9%)高于对照组(6.0%)。形态不规则在微小乳腺癌组所有恶性征象中发生率最高。结论纵横比> 1、微钙化、形态不规则是微小乳腺癌常见的声像图恶性征象,前两者特异性较高,后者敏感性较高。腺癌组所有恶性征象中发生率最高。结论纵横比>1、微钙化、形态不规则是微小乳腺癌常见的声像图恶性征象,前两者特异性较高,后者敏感性较高。Objective To explore the ultrasound manifestations of small breast cancer,and to summerize the typical malignant features of small breast cancer. Methods The ultrasonographic data of 105 patients with pathologically confirmed small breast cancer（less than 1 cm in di-ameter）were retrospectively analyzed,and the data of 100 patients with pathologically confirmed larger breast cancer（ 〉 1 cm）in the same period were compared,then the difference in ultrasonography was compared between these two groups. Results The incidence of malignant signs in pa-tients of larger breast cancer group was significantly higher than that of patients in small breast cancer group. The difference in irregular shape,as-pect ratio 〉 1,micro calcification,internal blood flow and the presence of metastasis in axillary lymph nodes between these two groups was statisti-cally significant. The incidence rate of aspect ratio 〉 1 in the experimental group was higher than that of control group. The highest incidence rate in all malignant signs in patients of experimental group was irregular shape. The specificity of micro calcification in diagnosis of small breast cancer was the highest. Conclusion Aspect ratio 〉 1,micro calcification and irregular shape are common malignant signs of small breast cancer,the for-mer two signs are highly specific,and the latter one is highly sensitive.
