摘  要:目的:探讨不同听障程度、不同干预年龄的语前聋儿童助听后听觉与言语能力发育的相关性,为患儿后期康复及助听效果评估提供参考。方法以23例语前聋患儿(男11例,女12例)为研究对象,根据患儿耳聋程度分为A组(重度耳聋组,13例)和B组(极重度耳聋组,10例),根据患儿干预年龄分为C组(≤2岁组,15例)和D组(>2岁组,8例)。使用听觉行为分级标准(categories of auditory performance,CAP)、言语可懂度分级标准(speech intelligibility rating,SIR)及有意义使用言语量表(meaningful use of speech scale,MUSS)评估患儿助听后的听觉及言语发育情况,对评估得分进行Spearman相关分析。结果3组患儿的CAP与SIR、CAP与MUSS、SIR与MUSS评估得分之间均存在显著相关性,相关系数rA分别为0.973、0.969、0.973(P<0.01),相关系数rB分别为0.926、0.862、0.886(P<0.01),相关系数rC分别为0.944、0.975、0.960,P<0.01,相关系数rD分别为0.881、0.902、0.868(P<0.01)。结论不同听障程度及不同干预年龄的语前聋儿童助听后听觉能力和言语能力均存在显著相关性,CAP、SIR、MUSS 3种问卷可联合用于听障儿童听觉言语能力的评估。Objective To explore the relationship between auditory and speech abilities in hearing-impaired children with different degrees of hearing loss or of different ages receiving intervention fitting, and to provide useful information for the auditory and speech rehabilitation and outcome assessment.MethodsA total of 23 children (11 boys and 12 girls) with pre-lingual hearing loss participated in this study. According to the degree of hearing loss, all subjects were divided into two groups: Group A (severe hearing loss, n=13) and Group B (profounds hearing loss, n=10). According to the age at hearing intervention, all subjects were divided into two groups: Group C (≤2 years old, n=15) and Group D (〉2 years old, n=8). The questionnaires of CAP, SIR and MUSS were used to evaluate the participants’ auditory and speech abilities after hearing intervention. The Spearman rank correlation analysis was applied in the statistical procedure.Results There was a significant correlation between CAP and SIR, CAP and MUSS, as well as SIR and MUSS in Group A,B,C and D respectively(P〈0.01).Conclusion There is a significant correlation between auditory and speech ability in children with different degrees of hearing loss or of different ages receiving intervention. It is recommended that CAP, SIR and MUSS be used in the comprehensive assessment of auditory and speech abilities in hearing-impaired children.
