白癜风患者中医体质临床研究

摘　　要：目的探讨白癜风患者中医体质特点,为临床辨病、辨证、辨体质结合治疗白癜风提供流行病学依据。方法对296例确诊为白癜风的患者进行问卷登记,调查内容包括性别、年龄、病程、中医体质类型、舌象、脉象等,并对获得数据进行统计学分析,总结白癜风患者中医体质特点及其与各因素间相关性。结果白癜风患者体质类型中病理性体质占82.16%,单纯平和质占17.84%,两组差异具有统计学意义（P〈0.01）。各种体质构成比差异具有统计学意义（χ^2=99.506,P=0.000）。不同性别、不同年龄段白癜风患者体质类型构成比差异均具有统计学意义（χ^2=25.008,P=0.002;χ^2=101.236,P=0.000）。不同病程白癜风患者体质类型构成比差异无统计学意义（χ^2=7.692,P=0.464）。白癜风患者各种舌象、脉象构成比差异具有统计学意义（舌质χ^2=392.381,P=0.000;舌苔χ~2=345.646,P=0.000;脉象χ^2=159.986,P=0.000）。舌象、脉象体征频数表中,舌质红或淡红、舌体胖大、舌苔白腻、脉弦细或沉细者居多。结论白癜风患者中医体质类型中以平和质居多,其次为气虚质、阴虚质等病理性体质。中医体质类型分布与白癜风患者性别、年龄因素间存在密切关联,而与病程长短无明显相关。Objective To investigate the characteristics of the constitution of the patients with vitiligo from traditional Chinese medical view,thus to provide epidemiological evidence for traditional Chinese medicine（TCM） treatment based on differential diagnosis of diseases,symptoms and signs,and constitutions.Methods Questionnaires were filled by 296 patients with vitiligo,which designed including the following items： sex,age,course of disease,somatotypes in TCM,tongue demonstrations,pulse tracings,etc.The data were statistically analyzed to summarize the relationship between somatotypes and above factors.Results The constituent ratio of the pathological somatotypes and balanced somatotypes was 82.16% and 17.84% respectively.There was significant difference between the two groups（P〈0.01）.The difference between the constituent ratio of various somatotypes patients with vitiligo has statistical significance（χ^2=99.506,P=0.000）.This significant difference also exists in different sex or ages（χ^2=25.008 P=0.002,χ^2=101.236,P=0.000,respectively） but not in different course of disease（χ^2=7.692,P=0.464）.There was significant difference between different tongue demonstrations and pulse tracings（body of the tongue： χ^2=392.381,P=0.000,coated tongue： χ^2=345.646,P=0.000 pulse tracings： χ~2=159.986,P=0.000,respectively）.Red or carmoisine in color and big or fat in shape were frequently seen in the body of the tongue,so was white and greasy fur in coated tongue.Sunken or wiry pulse with small pulses were most common in vitiligo patients（P〈0.01）.Conclusion The balanced constitution is the most common type of TCM somatotypes in vitiligo patients,followed by deficiency of vital energy,yin asthenia and other pathological constitution.The somatotypes of patients with vitiligo do have correlation with sex or ages but not the course of disease.
