膝关节镜清理术联合盐酸氨基葡萄糖治疗膝骨关节炎的临床疗效观察

摘　　要：目的探讨膝骨关节炎(KOA)行关节镜清理术后的常规治疗方案临床疗效。方法选择2013年1月-2015年4月收治的62例KOA患者为研究对象,将62例患者随机分为试验组和对照组,每组各31例。对照组单用关节镜下清理术,试验组在对照组治疗基础上加用口服盐酸氨基葡萄糖进行治疗。采用疼痛视觉模拟评分(VAS)及Lysholm膝关节功能评分方法,对术前及术后1周、4周、3个月、6个月患膝疼痛及关节功能状况进行评价。结果术前和术后1周两组VAS评分和Lysholm评分比较差异无统计学意义(P>0.05)。术后4周、3个月、6个月试验组VAS评分分别为(3.08±0.91)、(2.46±0.87)、(1.45±0.66)分,对照组VAS评分分别为(5.47±1.02)、(3.55±1.20)、(2.37±0.53)分;试验组Lysholm评分为(80.55±2.24)、(85.35±1.79)、(89.74±4.58)分,对照组Lysholm评分为(72.55±4.47)、(74.68±2.94)、(76.69±5.63)分;试验组与对照组VAS评分和Lysholm评分比较差异有统计学意义(P<0.05)。结论膝关节镜清理术对KOA的症状有较大改善,而术后口服盐酸氨基葡萄糖则可明显提高疗效。Objective To investigate the effect of arthroscopic debridement combined with oral glucosamine hydrochloride tablets in the treatment of knee osteoarthritis.Methods Sixty-two patients with knee osteoarthritis treated between January 2013 and April 2015 were chosen to be our research subjects.They were randomly divided into trial group（n=31） and control group（n=31）.The control group was treated with arthroscopic debridement,and the trial group was given glucosamine hydrochloride tablets for treatment,apart from arthroscopic debridement.We evaluated the clinical effects by visual analogue scale（VAS） score and Lysholm knee joint function score before,and 1 week,4 weeks,3 months and 6 months after surgery.Results One week after surgery,the VAS score and Lysholm knee joint function score were not significantly different between the two groups（P〉 0.05）.Four weeks,3 months and 6 months after surgery,the VAS score of the trial group was respectively 3.08±0.91,2.46±0.87,and 1.45±0.66,and was 5.47±1.02,3.55±1.20,and 2.37±0.53 in the control group;the Lysholm score of the trial group was 80.55±2.24,85.35±1.79,and89.74±4.58,respectively,and of the control group was 72.55±4.47,74.68±2.94,and 76.69±5.63.The VAS score and the Lysholm score of the trial group were both better than those of the control group（P〈 0.05）.Conclusion Arthroscopic debridement can alleviate the symptoms of knee osteoarthritis,and oral administration of glucosamine hydrochloride tablets after surgery has obvious effects.
