人工关节置换和股骨近端髓内固定治疗老年人股骨粗隆间骨折的对比探讨

摘　　要：目的:探讨人工关节置换和股骨近端髓内固定(PFNA)治疗老年人股骨粗隆间骨折的效果差异。方法:将81例老年人股骨粗隆间骨折患者的临床资料进行回顾性分析。结果:人工关节置换组38例老年人股骨粗隆间骨折患者,经人工关节置换术治疗,Harris髋关节功能评分为(89±8)分,优良率为94.74%;均明显高于PFNA内固定组,经比较,差异具有统计学意义(P<0.05)。手术时间、术中出血量、术后负重行走时间短均明显少于PFNA内固定组,经比较,差异具有统计学意义(P<0.05)。结论:人工关节置换术治疗老年人股骨粗隆间骨折患者具有手术时间短、创伤小、术中出血少、术后负重行走时间短,H arris髋关节功能评分高等优点,值得进一步推广应用。Objective To investigate the effect of artificial joint replacement and PFNA internal fixation in the treatment of in-tertrochanteric fracture in the elderly.Method The clinical data of 81 elderly patients with intertrochanteric fracture in our hospital from June 2013 to December 2015 were retrospectively analyzed.Results the artificial joint replacement group 38 cases of elderly patients with femoral intertrochanteric fracture,after artificial joint replacement therapy,Harris hip score was （89 ＆#177;8）,the excellent rate was 94.74%;PFNA was significantly higher than that in the fixed group,by comparison,the differences were statistically signif-icant （P 〈0.05）.The operation time,bleeding volume,crutches time early internal fixation group were significantly less than PF-NA,by comparison,the difference was statistically significant （P 〈0.05）.Conclusion Artificial hip replacement for the treatment of senile intertrochanteric fractures in patients with clinical efficacy exact,short bed time,postoperative fracture healing effect is good,worthy of further application.
