经尿道钬激光联合吡柔比星和丝裂霉素膀胱灌注治疗腺性膀胱炎的疗效观察

摘　　要：目的：对比经尿道钬激光联合吡柔比星和丝裂霉素膀胱灌注治疗腺性膀胱炎（CG）的临床疗效。方法：回顾性分析被诊断为CG,经尿道钬激光联合吡柔比星和丝裂霉素膀胱灌注治疗的患者共46例。随机分为两组,吡柔比星组（A组）26例,丝裂霉素组（B组）20例。比较两组患者的有效率和复发率。结果：46例患者均顺利完成手术,随访3~24个月,其中A组治愈14例,好转10例,未愈2例;B组治愈8例,好转5例,未愈7例。A组的有效率（92.3%）明显高于B组（65.0%）,差异有统计学意义（P〈0.05）;A组复发率（7.7%）明显低于B组（35.0%）,差异有统计学意义（P〈0.05）。结论：吡柔比星和丝裂霉素灌注后不良反应发生率低,均无严重术后并发症发生,患者可耐受,均可作为膀胱灌注的药物选择。但综合分析,CG术后联合吡柔比星膀胱灌注,有效率更高、复发率更低小,是治疗CG的理想化疗药物。Objective To compare the efficacy of transurethral holmium laser combined pirarubicin and mitomycin in the treat-ment of cystitis glandularis.Method We conducted a retrospective analysis on the patients who was diagnosed with cystitis glandu-laris.There were 46 patients who used transurethral holmium laser combined pirarubicin and mitomycin in the treatment of cystitis glandularis.Patients were randomly divided into two groups,there were 26 cases of pirarubicin group,mitomycin group had 20 ca-ses.We had compare on efficient and recurrence rate of the two groups.Results All 46 patients were successfully operated.Fol-lowed up for 3 months,the patients of pirarubicin group were cured 14 cases,improved in 10 cases,healed in 2 cases.The patients of mitomycin group were cured 8 cases,improved in 5 cases,healed in 7 cases.The efficiency of pirarubicin group （92.3%）was significantly higher than that of mitomycin group （65.0%）,the difference was statistically significant （P 〈0.05）.The relapse rate of pirarubicin group （7.7%）was significantly lower than that of mitomycin group （35.0%）,the difference was statistically signifi-cant （P 〈0.05）.Conclusion After the patients were perfused with pirarubicin and mitomycin,there are low incidence of adverse reactions and serious postoperative complications,and patients could tolerate the.However,there are higher efficiency and lower re-lapse rate using pirarubicin after operativing.So pirarubicin is an ideal medication in the treatment of cystitis glandularis.
