有限切开内固定结合外固定架治疗桡骨远端C型骨折疗效分析

摘　　要：目的：探讨有限切开内固定结合外固定架治疗桡骨远端C型骨折的疗效。方法：对采用有限切开内固定结合外固定架治疗桡骨C型骨折32例患者进行回顾性研究。根据AO分型,其中C1型8例,C2型11例,C3型13例,均为新鲜骨折,测定腕关节的活动范围和影像学指标,对腕关节功能进行Gartland-Werle评分。结果：随访6~8个月,其中1例因开放骨折,术后出现伤口皮肤坏死,经换药后治愈,其余均未见针道感染和神经、肌腱损伤,骨折均获痊愈。患侧腕关节功能的屈伸和尺偏范围显著小于正常腕关节,差异有统计学意义（P〈0.05）,旋前和旋后活动范围与正常腕关节比较,差异无统计学意义（P〉0.05）。掌倾角显著小于正常腕关节,差异有统计学意义（P〈0.05）,桡骨高度和尺偏角比较,差异无统计学意义（P〉0.05）。Gartland-Werley功能评分优良率为93.7%。结论：采用有限切开内固定结合外固定架的手术方法治疗桡骨远端C型骨折是一种损伤较小且安全有效的方法。Objective To evaluate the effect of limited internal fixation combined with external fixation in the treatment of AO type C distal radius fracture.Method A retrospective study was conducted in 32 patients who had been treated with limited internal fixation combined with external fixation.According to AO classification,C1 8 cases,C2 11 cases,C3 13 cases,Wrist movement range and radiographic index were measured and wrist function was evaluated by the Gartland -Werle criteria.Results After 6 ~8 months＆amp;nbsp;follows -up,Patients wrist dorsiflexion and ulnar deviation range was significantly less than the normal activities of the wrist （P 〈0.05）,there was no significant difference in the movement range of flexion,pronation,supination and radial deviation （P 〉0.05）, and the difference was no significant in the radial height and ulnar inclination between the treated wrist joint and the normal hand （P 〉0.05）.Nevertheless,the recorvery of thewrist extension,ulanr deviation and palm tilt was limeted,significantly smaller than the normal（P 〈0.05）.By the Gartlan -Werley criteria,the excellence rate was 93.7%.Conclusion Limited internal fixation com-bined with external fixator operation method is an effective,smaller damage and safe therapy for type C fracture of the distal radius.
