二尖瓣机械瓣膜置换同期行双极射频消融术治疗心房颤动的早期疗效

摘　　要：目的分析二尖瓣机械瓣膜置换同期行双极射频消融术治疗心房颤动的早期疗效。方法回顾性分析2011年8月至2014年8月,45例二尖瓣病变合并房颤患者在瓣膜置换同期行BRFA治疗房颤。并取同期同一医师单纯行二尖瓣置换手术52例作为对照。记录术前,术后当日,出院时,出院后1、3、6个月心电图,超声心动图等指标,比较其疗效。结果同期手术组出院时窦性心律39例,心房颤动消除率86.67%。术后给予胺碘酮治疗3个月。术后6个月,91.11%患者维持窦性心律、左心房、左心室径线较术前明显减小,左心室射血分数较术前明显增高,差异有统计学意义(P<0.05)。结论二尖瓣机械瓣膜置换同期施行双极射频消融术安全,简便,短期临床效果满意。Objective To analyze the early efficacy of bipolar radiofrequency ablation in the treatment of atrial fibrillation in patients with mitral valve replacement. Methods We reviewed and analyzed the BRFA treatment of atrial fibrillation in 45 patients with mitral valve disease complicated with atrial fibrillation during the same period of valve replacement from August 2011 to August 2014. And 52 patients with simple mitral valve replacement surgery by the same doctor in the same period were selected as the control group. We recorded preoperative,postoperative day,discharge,1、3、6 months after discharge,ECG,echocardiography and other indicators,and compared the efficacy. Results At the same time,39 patients with sinus rhythm from the surgery group were discharged,and the elimination rate of atrial fibrillation was 86. 67%. And after the surgery,they were treated with amiodarone for 3 months. 6 months after surgery,91. 11% patients maintaining sinus rhythm,left atrium,left ventricular diameter were significantly decreased,the left ventricular ejection fraction was significantly higher than before,and the difference was statistically significant（ P〈0. 05）. Conclusion Mitral valve replacement with mechanical valve replacement is safe and convenient,and the short-term clinical results are satisfactory.
