Crede手法在神经源性膀胱康复护理中的疗效观察

摘　　要：目的观察Crede手法在神经源性膀胱康复护理中的应用疗效。方法神经源性膀胱患者共57例,随机分为抗胆碱药物使用、间歇导尿配合Crede手法组(治疗组)29例及抗胆碱药物使用配合间歇性导尿组(对照组)28例。在治疗前、后分别采用尿流动力学分析及下尿道综合症状(lower urinary tract symptoms,LUTS)进行疗效评估。结果治疗4周后,两组患者残余尿量、最大逼尿肌压及尿道综合症状评分均较治疗前有明显下降(P<0.05),且治疗组较对照组下降更显著(P<0.05);两组患者最大膀胱测压容积较治疗前有明显增加(P<0.05),且治疗组较对照组增加更显著(P<0.05);治疗组总有效率优于对照组(P<0.05)。治疗过程中无不良反应事件的发生。结论 Crede手法配合间歇性导尿可有效改善神经源性膀胱排尿情况和尿流动力学,可作为安全有效的治疗手段之一,适合临床护理推广应用。Objective To observe the therapeutic effect of Crede technique in the treatment of neurogenic bladder rehabilitation nursing. Methods A total of 57 cases of patients with neurogenic bladder were randomly divided into anticholinergic drug use、intermittent catheterization with Crede maneuver group（ treatment group） 29 cases and anticholinergic drugs combined with intermittent catheterization group（ control group） 28 cases. Before and after the treatment,the efficacy was evaluated by the analysis of the urine flow dynamics and the lower urethral syndrome（ lower urinary tract symptoms,LUTS）. Results After 4 weeks of treatment,the residual urine volume,the maximum detrusor pressure and the urethral syndrome score of two groups of patients were decreased significantly than those before treatment（ P〈0. 05）,and compared with the control group,the treatment group decreased significantly（ P〈0. 05）. The maximum bladder pressure volume of the two groups were significantly increased,compared with those before treatment（ P〈0. 05）,and the treatment group was significantly higher than the control group（ P 0. 05）. The total effective rate of the treatment group was better than that of the control group（ P〈0. 05）. No adverse events occurred during the treatment. Conclusion Crede maneuver with intermittent catheterization can effectively improve the neural source of bladder and urine flow dynamics,as a safe and effective means of treatment,and it is suitable for the popularization and application of clinical nursing.
