中药煎剂灌肠治疗急性放射性直肠炎临床研究

摘　　要：目的:探讨中药煎剂灌肠治疗急性放射性直肠炎治疗放射性直肠炎的临床疗效。方法:将72例患者随机分为观察组和对照组,每组36例。治疗组采用清热解毒、祛风除湿法,自拟中药煎剂(防风15 g,络石藤30 g,白花蛇舌草30 g,海桐皮15 g,红藤15 g,赤芍15 g,牡丹皮15 g,白鲜皮30 g,苦参30 g,白头翁15 g)保留灌肠治疗;对照组给予甲硝唑灌肠治疗。两组均以10 d为1个疗程,共治疗2个疗程。观察两组患者治疗前、治疗1个疗程、治疗2个疗程的直肠黏膜状况、卡氏(Karnofsky,KPS)评分、症状积分以及不良反应。结果:治疗后治疗组患者肠黏膜状况、KPS评分和症状积分均优于对照组,差异有统计学意义(P<0.05)。结论:自拟中药煎剂保留灌肠治疗急性放射性直肠炎疗效显著。Objective： To examine the clinical efficacy of a Chinese herbal medicine decoction used as an enema in treatment of Radiation Proctitis. Methods： 72 patients were randomly divided into observation group and control group,with 36 cases in each group. The treatment group received enema treatment with TCM therapy,which functions in Heat clearing,detoxification,as well as dispelling Wind and Dampness. The composition of the prescription is like this： Saponikovia 15 g,Contact Ishido 30 g,Diffusa 30 g,Pittosporum Skin 15 g,Sargent Gloryvine 15 g,Red Peony Root 15 g,Peony Bark 15 g,Dictamni 30 g,Sophora 30 g,Pulsatilla15 g. The control group received Metronidazole as enema. The two groups experienced 2 course of treatment for 20 d. The rectal mucosa conditions,Karnofsky（ KPS） scores,symptom scores,and adverse reactions of the both groups were observed before the treatment,after one course of treatment and after 2 courses of treatment. Results： After treatment,the rectal mucosa conditions,KPS scores and symptom scores of the treatment group were better than those of the control group,the differences were statistically significant（ P 〈0. 05）. Conclusion： This Chinese herbal medicine decoction used as an enema in treatment of acute Radiation Proctitis has a significant clinical effect.
