中药逆转肿瘤多药耐药的相关研究

摘　　要：多药耐药是化疗失败的关键因素。西药作为逆转剂,其不良反应往往难以耐受,并且作用靶点相对比较单一,价格昂贵,临床应用受到很大限制。中药逆转剂已经逐渐成为研究的热点,主要从单味中药和中成药提取物抗耐药两方面进行研究。单味中药主要研究川芎、榄香烯、全蝎、防己、冬凌草、苦参、砒霜、薏苡仁、鸦胆子,中成药主要研究生脉注射液、复方三根制剂、浙贝母散剂、拮新康。作者认为,中药逆转剂靶点相对广泛,价格低廉,不良作用较少,同时可以将中医的辨证思维融入整个治疗之中,根据患者证候来选择相应的中药逆转剂,但是由于耐药机制十分复杂,单一针对某一个机制的临床疗效并不是很好,一种化疗药物的耐药产生涉及到多个耐药机制的参与,如多种耐药基因、蛋白表达的改变,而这些研究仍处于基础研究阶段,并没有应用于临床,在体内的作用机制也均是凭一些临床经验进行判断。同时,某些中药的逆转机制还不是很明确。Multi-drug resistance is a key factor in failure of chemotherapy. The application of western medicine,acting as a reversal agent,is very restricted in clinical practices due to the strong adverse reactions,the difficulties in toleration,the single target functions and the relatively expensive prices,and so on. Currently,Chinese medicine being reversal agents has gradually become one of the research focuses. This study is carried out in aspect of the anti-drug resistance effect of a single TCM herb or the herb extracts.The single herbs mainly include Ligusticum,Elemene,Scorpions,Tetrandra Rubescens,Matrine,Arsenic,Coixenolide,Brucea,while Chinese patent medicine include Shengmai Injection,Three Root Compound Preparations,Fritillaria Powders and Jiexinkang in this research. This study suggests that there are many targets hit by traditional Chinese medicine,and with low prices,fewer side effects and the dialectical thinking,it is reasonable for TCM being integrated into the entire treatment of caner in the future. Of course,further studies need to be conducted in the mechanism of multi-drug resistance of TCM.
