噻托溴铵联合孟鲁司特钠治疗慢性阻塞性肺疾病患者的疗效分析

摘　　要：目的:分析噻托溴铵与孟鲁司特钠联用治疗慢性阻塞性肺疾病患者的疗效。方法:选取2015年6月—2016年6月间收治的慢性阻塞性肺疾病患者120例;将其分为孟鲁司特钠组、噻托溴铵组和联用组,每组40例;分别给予单用孟鲁司特钠,单用噻托溴铵,和孟鲁司特钠与噻托溴铵联用治疗;比较3组患者治疗后的总有效率、治疗前后肺功能各指标(FEV1、PEF和FEV1/FVC百分率)、呼吸困难症状评分值及6 min步行试验(6 MWT)评分值变化情况以及治疗期间药物不良反应的发生情况。结果:联用组患者治疗后的总有效率明显高于孟鲁司特钠组和噻托溴铵组(P<0.05);治疗后FEV1、PEF、FEV1/FVC百分率均高于孟鲁司特钠组和噻托溴铵组(P<0.05);治疗后呼吸困难症状评分值及6 MWT评分值均低于孟鲁司特钠组和噻托溴铵组(P<0.05);3组患者治疗期间服药所致不良反应的发生率经比较其差异无统计学意义(P>0.05)。结论:采用噻托溴铵与孟鲁司特钠联用治疗方案,能够有效改善慢性阻塞性肺疾病患者呼吸困难症状和肺功能各指标,临床疗效较显著,安全性较高。Objective： To analyze the clinical efficacy in patients with chronic obstruct pulmonary disease（COPD） by using combination of tiotropium bromide and montelukast sodium. Methods： 120 cases of patients with COPD were divided into montelukast group, tiotropium bromide group, and combining group. They were treated with montelukast, tiotropium bromide, and their combination respectively. The total effective rate, FEV1, PEF, FEVI/FVC, the scores of breathing difficulties, the scores of 6 MWT and adverse drug reactions were compared between the two groups. Re- sults： The total effective rate at the end of the course for the combining group were higher than that of montelukast group and tiotropium bromide group（P〈 0.05）. The levels of FEV 1, PEF and FEV 1/FVC for the combining group after treatment were higher than that of montelukast group and tiotropium bromide group（P〈 0.05）. The scores of breathing difficulties and the scores of 6 MWT for the combining group after treatment were lower than those of montelukast group and tiotropium bromide group（P〈 0.05）. There were no significant differences on the rate of adverse reactions for the three groups（P〉0.05）. Conclusion： It can improve the scores of breathing difficulties and lung function for the pa- tients with COPD with tiotropium bromide. When combined with montelukast, it has distinct clinical curative effect, with safety. It can be used as a clinical treatment optimization scheme.
