锝99-甲氧基乙腈亲肿瘤显像联合血清肿瘤标志物诊断肺结节良恶性的临床价值

摘　　要：目的研究锝99-甲氧基乙腈(^(99m)Tc—MIBI)亲肿瘤显像联合血清肿瘤标志物(NSE、CYFRA21—1)在肺部结节诊断中的意义。方法我院54例患者经电子计算机断层扫描(computed tomography,CT)发现肺部结节,均行^(99)Tc^m-MIBI亲肿瘤显像检查及血清肿瘤标志物检测,分别计算这两种方法以及两种方法联合应用的灵敏度、特异性、准确性、阳性预测值、阴性预测值,并用统计学方法将三者的结果进行逐一比较分析。结果两者联合应用的准确性和阳性预测值均高于^(99)Tc^mMIBI亲肿瘤显像和肿瘤标志物的测定结果,有统计学意义(P<0.05);两者联合的特异度高于肿瘤标志物的测定结果,差异有统计学意义(P<0.05),两者联合与^(99)Tc^m-MIBI亲肿瘤显像之间的特异度差异无统计学意义(P>0.05);灵敏度和阴性预测值三者之间差异无统计学意义(P>0.05)。结论 ^(99)Tc^m-MIBI亲肿瘤显像联合血清肿瘤标志物测定在鉴定肺部结节性质方面具有重要的临床意义。Objective To evaluate the significance of 99Tcm-MIBI imaging combined with serum tumor markers （NSE, CYFRA21-1） in diagnosis of pulmonary nodules. Methods 54 patients who were detected pulmonary nodules by Computed Tomography （CT） were involved in this study. All of them underwent 99Tcm-MIBI imaging and serum tumor markers testing. The sensitivity, specificity and accuracy of two methods were calculated respectively and the sensitivity, specificity,accuracy, positive predictive value, negative predictive value of combined application of two methods were also calculated. Results The accuracy rate and positive predictive value of 99Tcm-MIBI imaging combined with serum tumor markers were higher than that of alone detection （P〈0.05）. The specificity of the combination detection was higher than that of tumor marker detection （P〈0.05）. There was no significantly difference on the specificity between combination detection and 99Tc m-MIBI imaging （P〉0.05）. The sensitivity, negative predictive value among combination, 99Tc m-MIBI imaging and tumor marker detection had no significance difference （P〉0.05）.Conclusion 99Tcm-MIBI imaging combined with serum tumor markers has great clinical significance in identifying pulmonary nodules.
