类风湿性关节炎寒湿痹阻证患者血清和关节液IL-1、IL-6、Blys的表达研究

摘　　要：目的:检测类风湿性关节炎(rheumatoid arthritis,RA)寒湿痹阻证患者血清和关节液的白介素-1(interleukin-1,IL-1)、白介素-6(interleukin-6,IL-6)和B淋巴细胞刺激因子(Blys)的表达水平,分析IL-1、IL-6、Blys和血沉(erythrocyte sedimentation rate,ESR)、C反应蛋白(creactive protein,CRP)、DAS28之间的关系,研究IL-1、IL-6、Blys在RA发病中的作用及临床意义。方法:采用ELISA法检测50例RA寒湿痹阻型患者外周血清,30例RA寒湿痹阻型患者关节液、30例骨关节炎患者关节液和50例健康对照组血清IL-1、IL-6、Blys的表达,同时常规检测ESR、CRP,利用SPSS17.0统计软件进行统计学分析。结果:RA寒湿痹阻型患者外周血血清中IL-1、IL-6、Blys表达水平显著高于健康对照组,差异具有统计学意义(P<0.05);RA患者关节液中IL-1、IL-6、Blys的表达水平显著高于外周血清组及骨关节炎关节液组,差异具有统计学意义(P<0.05)。RA寒湿痹阻型患者外周血血清和关节液中Blys的表达与ESR、CRP、DAS28呈正相关,差异具有统计学意义(P<0.05),而IL-1、IL-6的表达与ESR、CRP、DAS28无相关性,差异无统计学意义(P>0.05)。结论:RA寒湿痹阻型患者外周血和关节液中IL-1、IL-6、Blys的异常升高可能参与了RA的发病。Objective：To determine the expression of IL-1,IL-6 and Blys in serum and joint fluid in patients with rheumatoid arthritis （RA） due to cold damp obstruction, analyze the relationship among IL-I, IL-6,Blys and ESR, CRP and DAS28 ,and study the action and the clinical significance of IL-1, IL-6 and Blys in the attack of RA. Methods：The expression of IL-I, IL-6 and Blys in peripheral serum of totally 50 patients with RA due to cold damp obstruction, joiut fluid of 30 patienis with RA due to cold damp obstruction,joint fluid of 30 patients with osteoarthritis,serum of 50 healthy patients were determined by ELISA. The ESR and CRP were also measured and statistical software of SPSS 17.0 was applied to make statistical analysis, Results：The expres- sion levels of IL-1, IL-6 and Blys in serum in patients with RA due to cold damp obstruction were significantly higher than these in healthy control group;the difference had statistical significance （ P 〈 0.05 ）. The expression levels of IL-1, IL-6 and Blys in joint fluid in patients with RA were significantly higher than these in the peripheral serum and those in the joint fluid in patients with osteoarthritis;the difference had statistical significance（ P 〈 0.05 ）. Among patients with RA due to cold damp obstruction, the expression of Blys in the peripheral serum and joint fluid was positively associated with ESR, CRP and DAS28, with statistical significance（ P 〈 0.05）. While the expression of IL-I and IL-6 is not associated with ESR, CRP and DAS28, without statistical significance （ P 〉 0. 05 ）. Conclusion ： The abnormal rise of the IL-1, IL-6 and Blys in serum and joint fluid in patients with RA due to cold damp obstruction may be related with the attack of RA.
