中西医结合治疗妊娠期糖尿病41例

摘　　要：目的:观察中西医结合治疗妊娠期糖尿病的临床疗效。方法:选取2013年3月-2014年10月本院诊治的妊娠期糖尿病孕妇82例作为研究对象,随机分为对照组和研究组,每组41例。对照组孕妇实施健康教育、饮食控制和运动锻炼,研究组在对照组治疗的基础上加用泰山磐石散合玉液汤治疗。观察并记录两组孕妇治疗前后餐后2h血糖、空腹血糖、糖化血红蛋白及BMI的变化情况,并发症发生情况;观察并记录两组婴儿出生结局。结果:治疗后两组孕妇餐后2 h血糖、空腹血糖、糖化血红蛋白及BMI与治疗前比较,差异有统计学意义(P<0.05),且研究组治疗后餐后2 h血糖、空腹血糖、糖化血红蛋白及BMI优于对照组(P<0.05);研究组出现子痫前期、羊水过多、产褥期感染及早产并发症的发生率显著低于对照组(P<0.05);两组治疗后婴儿围产期死亡情况比较,差异无统计学意义(P>0.05);研究组巨大儿和婴儿窒息发生率与对照组比较,差异有统计学意义(P<0.05)。结论:中西医结合治疗妊娠期糖尿病疗效显著,既可控制血糖水平波动幅度,还可以改善孕妇分娩结局,确保母婴安全。Objective ： To observe the clinical effects of integrative Chinese and western medicine on gestation complicated with diabetes. Methods：Totally 82 patients with gestation complicated with diabetes hospitalized between March,2013 and October,2014 were randomly divided into control group and research group,with 41 cases in each group. Control group were given health educa- tion,diet control and exercise training while research group were added with Taishan Panshi Powder combined with Jade Humor Decoction for the treatment. The changes of blood glucose 2 h after diet.fasting blood glucose,glycosyla）ed hemoglobin and BMI as well as the occurrence of the complications in both groups were observed and recorded. The birth outcome of the infants in both groups were observed and recorded. Results：The condition of blood glucose 2 h after diet, fasting blood glucose, glyeosylated he- moglobin and BMI after the treatment was better than that before treatment; the difference had statistical significance （ P 〈 0.05 ）, and the levels of blood glucose 2 h ＇after diet. fasting blood glucose, glycosylated hemoglobin and BMI in research group were better than these in control group （ P 〈 0.05 ）. The occurrence rate of the complications of preeclampsia, polyhydramnios, puerperium in- fection and premature birth was significantly lower than that in control group（ P 〈 0.05 ）. The difference in the death conditon dur- ing the perinatal period between the two groups had no statistical significance（ P 〉 0.05 ）. The difference in the occurrence rates of fetal macrosomia and infantile apnea between the two groups had statistical significance （ P 〈 0.05 ）. Conclusion ： Integrative Chinese and western medicine treatment has remarkable therapeutic effects on gestation complicated with diabetes ,which can not only control the fluctuation range of the blood glucose level but also improve the delivery outcome of the pregnant women and ensure maternal and child＇s safety.
