通痹消炎膏外敷结合同理心护理干预治疗膝骨关节积液132例

摘　　要：目的:观察通痹消炎膏外敷结合同理心护理干预治疗膝骨关节积液的临床疗效。方法:将264例膝骨关节积液患者随机分为对照组和观察组,每组各132例。对照组采用口服盐酸氨基葡萄糖胶囊及从关节腔抽液后冲洗等常规治疗方法,观察组在对照组的基础给予通痹消炎膏外敷并结合同理心护理干预。观察两组患者的临床疗效及治疗前后VAS、WOMAC评分、膝关节腔积液透明质酸(hyaluronic acid,HA)、白细胞介素-1β亚型(interleukin-1β,IL-1β)的含量。结果:两组患者治疗后HA、IL-1β含量优于治疗前,且观察组优于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后WOMAC和VAS评分较治疗前明显好转,且观察组优于对照组,差异具有统计学意义(P<0.05)。对照组有效率为93.18%,观察组有效率为99.24%,两组有效率比较,差异具有统计学意义(P<0.05)。结论:通痹消炎膏外敷结合同理心护理干预能有效治疗膝关节积液,疗效显著,其作用机制可能是通痹消炎膏外敷吸收后可增加关节腔内HA合成、抑制IL-1β分泌,结合同理心干预更能提高患者战胜疾病的信心而起到良好的治疗作用。Objective ：To observe the clinical effects of external application of Obstruction-Unblocking and Inflammation-Eliminating Plaster combined with empathetic nursing on hydrops of knee joints. Methods ：Totally 264 patients with hydrops of knee joints were randomly divided into control group and observation group, with 132 cases in each group. Control group were orally given Hy- drochloric Acid Glucosamine Capsules and conventional treatment of extracting fluid from the articular cavity and then washing, while observation group were added with external application of Obstruction-Unblocking and Inflammation-Eliminating Piaster combined with empathetic nursing. The clinical effects, scores of VAS and WOMAC, contents of HA and IL-1β in both groups were observed. Results ：After the treatment, the contents of HA and IL-1β as well as scores of VAS and WOMAC in both groups got obviously improved, and the improvement in observation group was better than that in control group ; the difference had statisti- cal significance （P 〈 0.05）. The effective rate was 93.18% in control group and 99.24% in observation group;the difference had statistical significance（ P 〈 0.05 ）. Conclusion ： External application of Obstruction-Unblocking and Inflammation-Eliminating Piaster combined with empathetic nursing can effectively treat the hydrops of knee joints. The mechanism may be the absorption of the effective components m Obstruction-Unblocking and Inflammation-Eliminating Plaster increasing the synthesis of HA and restraining the secretion of IL-1β. And with the combination of empathetic nursing, the confidence of the patients fighting against the disease gets enhanced.
