PDCA循环管理对降低护理人员锐器伤发生率的影响

摘　　要：目的 探讨PDCA（Plan,Do,Check,Action）循环管理在预防护理人员发生锐器伤中的应用及意义,通过持续改进措施,降低护理人员锐器伤的发生率。方法 随机选择2012年1月~2014年6月132例护理人员在不同干预期发生锐器伤情况,对应用PDCA循环前后主管护师、护师、护士锐器伤的发生率及不同干预期锐器伤种类、次数、发生率进行统计分析。本研究以2012年1月~10月为干预前组,2012年11月~2013年8月为干预中组,2013年9月~2014年6月为干预后组。结果 应用PDCA循环管理模式后主管护师、护师、护士锐器伤发生率均显著降低,锐器伤发生率从干预前的21.21%降低到干预后期的3.79%,其中玻璃安瓿割伤从干预前的6.82%降低到干预后期的1.52%,注射器针头刺伤从干预前的6.06%降低到干预后期的0.76%,手术刀片、缝针从干预前的3.03%降低到干预后期的0.76%,处置医疗废物从干预前的5.30%降低到干预后期的0.76%。结论运用PDCA循环管理模式能够规范护理人员操作行为,显著降低护理人员锐器伤的发生率,稳定护理人员队伍,值得推广。Objective To explore the application and significance of PDCA（Plan, Do, Check, Action）cycle management in preventing nursing staff from sharp instrument injury so as to reduce the the incidence of sharp instrument injury by taking continuous quality improvement program. Methods A total of 132 nurses from Jan 2012 to Jun 2014 were investigated randomly on sharp instrument injury in different intervention period, the instrument injury incidence of the chief nurse and the senior nurse as well as the primary nurse around applying PDCA cycle management were analyzed,meanwhile the types, times and the incidence of the sharp injury in different intervention period were analyzed. In this study the nursing staff from Jan to Oct 2012 were defined as before intervention group, from Nov 2012 to Aug 2013 were defined as interim intervention group and from Sep 2013 to Jun 2014 were defined as late intervention group. Results The incidence of sharp instrument injury among nursing staff had a significant decline from 21.21% before intervention to 3.79% after intervention by applying PDCA cycle management, including the cuts of glass ampule were declined from 6.82% to 1.52%, the punctures of syringe needle were declined from 6.06% to 0.76%, the surgical blades and stitches were declined from 3.03% to 0.76%, disposing medical waste was declined from 5.30% to 0.76%. Conclusion PDCA cycle management model is worth generalizing and applying as the nursing staff＇s operating procedures can be standardize, the incidence of sharp instrument injury among nursing staff is declined, and nursing troop can be stabilized.
