数字减影造影与Garden分型评测股骨颈骨折闭合复位加压空心钉内固定疗效的比较

摘　　要：目的：探讨数字减影造影与Garden分型评测股骨颈骨折闭合复位加压空心钉内固定疗效的比较。方法：选取2013年3月-2015年3月在本院接受治疗的股骨颈骨折患者共60例,按照随机数字表法分为数字减影造影组和Garden分型组,每组30例。数字减影造影组依据数字减影造影检查结果行股骨颈骨折闭合复位加压空心钉内固定治疗,Garden分型组依据Garden分型结果行股骨颈骨折闭合复位加压空心钉内固定治疗。观察两组的分型结果、术后股骨头坏死发生率及不良反应。结果：数字减影造影组Ⅰ型占20.0%,Ⅱ型占26.7%,Ⅲ型占53.3%。Garden组Ⅰ型占16.7%,Ⅱ型占33.3%,Ⅲ～Ⅳ型占50.0%;数字减影造影组Ⅰ型股骨头坏死发生率为0,Ⅱ型为0,Ⅲ型为100%;Garden组Ⅰ型股骨头坏死发生率为0,Ⅱ型为10.0%,Ⅲ～Ⅳ型为100%。两组各分型股骨头坏死发生率比较,差异均无统计学意义（P〉0.05）;两组均未出现不良反应。结论：数字减影造影技术与Garden分型用于股骨颈骨折患者的评测中,其结果具有一致性,但数字减影造影技术更具有直观性,能够使医师对股骨颈骨折患者的治疗效果及其预后效果进行有效判断,因而可以更好地指导临床治疗。Objective：To compare the efficacy of digital subtraction angiography and Garden typing in the treatment of femoral neck fracture with closed reduction and internal fixation with compression screw fixation.Method：A total of 60 cases of femoral neck fractures treated in our hospital from March 2013 to March 2015 were randomly divided into digital subtraction angiography group and Garden group,30 cases in each group.Digital subtraction angiography group on the basis of digital subtraction angiography examination results of femoral neckfractures by closed reduction and cannulated compression hollow screw internal fixation,Garden group on the basis of the garden,the type classification results of the femoral neck fractures by closed reduction and cannulated compression hollow screw internal fixation.The genotyping results,femoral head necrosis after operation and the incidence of adverse reactions of two groups were compared.Result：Digital subtraction angiography group type Ⅰ accounted for 20.0%,type Ⅱ accounted for 26.7%,type Ⅲ accounted for 53.3%.Garden group type Ⅰ accounted for 16.7%,type Ⅱ accounted for 33.3%,type Ⅲ-Ⅳ accounted for 50.0%.Digital subtraction angiography group type Ⅰ avascular necrosis of the femoral head occurrence rate was 0,the incidence of type Ⅱ femoral head necrosis rate was 0 and type Ⅲ osteonecrosis of the femoral head occurred rate was 100%.Garden group type Ⅰ in femoral head necrosis rate was 0.Type Ⅱ femoral head necrosis rate was 10.0%,type Ⅲ-Ⅳ osteonecrosis of the femoral head occurred rate was 100%.Necrosis of the femoral head type in two groups were compared,there was no significant difference（P〈0.05）.Two groups showed no adverse reaction.Conclusion：Digital subtraction angiography technology and garden type used in the evaluation of patients with femoral neck fracture,and the results have consistency,but digital subtraction angiography technique is more intuitive to enable physicians to femoral neck fracture patients of therapeutic effect and
