单纯抗凝与静脉溶栓治疗在肠系膜静脉血栓治疗预后对比分析

摘　　要：目的探讨单纯抗凝与静脉溶栓治疗在肠系膜静脉血栓患者预后影响。方法回归性分析我科自2010年1月-2015年8月收住的肠系膜静脉血栓患者的临床资料,对其预后进行对比分析。结果共纳入21例患者,其中11例采用低分子肝素单纯抗凝治疗,另10例患者采用尿激酶溶栓联合抗凝治疗,两组患者均治愈,两组平均住院天数分别为20.2±4.1 d、20.7±3.2 d,无统计学差异(t=3.22 P=0.75);急性组患者平均住院天数19.1 d,亚急性组患者平均住院天数24.6 d,两组差异有统计学意义(t=3.86 P=0.001);溶栓组出现消化道出血4例,抗凝组无患者出现并发症(x2=5.435 P=0.02)差异有统计学意义,无其他并发症发生。结论单纯抗凝与溶栓在肠系膜静脉血栓治疗中效果无明显差异,但溶栓风险较抗凝高,其治疗的重点是早期发现并诊断,早期给予治疗。Objective Aim to discuss the prognoses effect of patients with mesenteric venous thrombosis between simple anticoagulation and intravenous thrombolysis therapy. Methods Regression analysis the clinical data of patients with mesenteric venous thrombosis who was hospitalized between January of 2010 and August of 2015, and the prognoses was comparatively analyzed. Results Participants included 21 patients, 11 cases with low molecular heparin anticoagulant therapy, the other 10 patients using urokinase thrombolysis with anticoagulant therapy. Two groups of patients were cured. Two groups of average hospitalization days were 20.2 ± 4.1days and 20.7 ± 3.2days, there was no significant difference among the groups （t=3.22 P =0.75）; Patients with acute group the average hospitalization days was 19.1 days, and subacute group average hospitalization days, wes 24.6 days, there was statistical difference between two groups （t=3.86 P=0.001）. 4 cases in thrombolysis group presented gastrointestinal bleeding, there was significant difference of patients in anticoagulant group. No other complications occurred. Conclusion There are no obvious difference between pure anticoagulation and thrombolysis effect in the mesenteric vein thrombosis treatment, but the risk of anticoagulant is higher than that of thrombolysis, the point of treatment is to find disease early and cure early.
