清瘿化痰汤治疗桥本甲状腺炎肝郁痰凝证临床观察  

Clinical Observation on Qingying Huatan Decoction in Treating Hashimoto's Thyroiditis of Liver Stagnation and Phlegm Coagulation Type

摘　　要：目的 :观察清瘿化痰汤治疗桥本甲状腺炎肝郁痰凝证的疗效。方法 :将76例符合桥本甲状腺炎肝郁痰凝证患者分为治疗组38例、对照组38例。对照组西药常规治疗,治疗组在对照组治疗基础上加用清瘿化痰汤,疗程为3个月。治疗前后分别评价中医证候评分、甲状腺功能、甲状腺自身抗体滴度、甲状腺峡部厚度等。结果:治疗后中医证候评分、甲状腺自身抗体滴度、甲状腺峡部厚度都有所改善,但治疗组改善情况明显优于对照组(P<0.01)。甲状腺功能两组变化均不明显。结论:清瘿化痰汤在改善中医证候、甲状腺自身抗体滴度和甲状腺峡部厚度等方面均有显著疗效,该方能调节机体免疫功能,对自身免疫因素引发的桥本甲状腺炎具有明确治疗效果。Objective ：To observe the curative effect of Hashimoto＇s thyroiditis of liver stagnation and phlegm coagulation type treated with Qingying Huatan decoction. Methods：There were 76 cases with Hashimoto＇s thyroiditis of liver Qi stagnation and phlegm coagulation type divided into the treatment group（38 cases） and the control group（38 cases）. In the control group,patients were given conventional treatment of western medicine,while in the treatment group,patients were given conventional treatment of western medicine and Qingying Huatan decoction. The treatment lasted for 3 months. Before and after treatment,traditional Chinese medicine （TCM） syndrome score, thyroid function, thyroid autoantibody titers, thyroid isthmus thickness were compared. Results ：After the treatment,TCM syndrome score,thyroid autoantibody titers,thyroid isthmus thick- ness were improved,the effects of the treatment group were better than those of the control group（P〈0.05）, but there were no obvious changes of thyroid function in the 2 groups. Conclusion：Qingying Huatan decoc- tion showed obvious effect on improving TCM syndrome,thyroid autoantibody titers and thyroid isthmus thickness,indicating that the decoction can regulate the immune function,and is effective for the Hashimoto＇s thyroiditis caused by autoimmune factors.
