应变率技术检测代谢综合征患者左室局部心肌功能的临床研究

摘　　要：目的探讨应变率技术检测代谢综合征(MS)患者左室局部心肌功能的可行性。方法选择66例符合MS诊断标准的成年人患者,分为MSⅠ组22例,MSⅡ组22例;22例健康成年人为对照组。采用二维斑点追踪技术,应用QLab在机分析软件检测患者局部左心室纵向、圆周、径向应变率参数。结果 MSⅠ组、MSⅡ组左心室纵向、圆周峰值应变率参数较对照组明显减低(P<0.05),MSⅠ组各节段较MSⅡ组均减低(P<0.05);MSⅠ组及MSⅡ组左心室径向峰值应变率参数与对照组比较部分节段明显减低(P<0.05),MSⅠ组二尖瓣水平后间隔应变率较MSⅡ组明显减低(P<0.05);ROC曲线分析显示,左室局部纵向收缩期峰值应变率对MS心肌病变均有一定诊断准确性,其最佳诊断分界点分别为-1.29/s。结论应用应变率技术可发现MS患者左室局部心肌形变能力,且左室局部纵向应变率可用于MS患者早期心肌病变的检测指标。Objective To detect regional of left ventricular myocardial function in patients with metabolic syndrome （ Ⅰ and Ⅱ group） using Strain Rate imaging（SRI）. Methods 66 patients with MS were enrolled in the study. MS Ⅰ group （n = 22） and MS Ⅱ group （n = 22） ;22 healthy adults as control group. SRI was used to detect strain rate. Qlab online analysis software was used to analyze each group. Results Compared with control group,left ventricular regional longitudinal and circumferential peak strain rate（SRS） were all declined significantly in MS Ⅰ and MS Ⅱ group （ P 〈 0.05 ）. Comparison between MS Ⅰ group and MS Ⅱ group, SRS were all declined significantly in MS Ⅰ group （P 〈 0.05 ）. Compared with control group, left ventricular regional peak radial strain rate was obviously reduced in part of segment of left ventricular in MS Ⅰ and MS Ⅱ group （ P 〈 0.05 ） ; Comparison between MS Ⅰ group and MS Ⅱ group, SRS was obviously reduced in mitral valve of after left ventricular interval in MS Ⅰ group （ P 〈 0.05 ）. ROC curve analysis showed that the curve of left ventricular regional longitudinal peak strain rate had certain diagnostic accuracy for MS I group. The best diagnostic cut - off point of them was - 1.29/s. Conclusion SRI can detect early regional left ventricular myocardial function dysfunction.
