摘 要:目的 观察超脉冲CO2点阵激光联合药物注射治疗增生性瘢痕的临床疗效.方法 对55例增生性瘢痕患者采用超脉冲CO2点阵激光扫描(CW,Active FX、Deep FX、SCAAR FX模式),治疗后采用美宝烧伤湿润膏外涂2周,治疗周期间隔3个月,治疗疗程3-8次;激光治疗间隔期间根据瘢痕情况行复方倍他米松与5-Fu联合注射治疗.比较患者治疗前后瘢痕疼痛、瘙痒指数、创面愈合时间、色素沉着发生率,并用温哥华瘢痕量表进行评分.结果 患者瘢痕的临床疼痛瘙痒感明显好转,温哥华瘢痕量表评分和色素沉着发生率均优于治疗前,差异均有统计学意义(P<0.05).结论 超脉冲CO2激光点阵疗法联合药物注射可有效治疗增生性瘢痕.Objective To observe the therapeutic effects of ultra-pulsed CO2 fractional laser eotnbined with drug injection therapy in the treatment of hypertrophic scar. Methods A total of 55 subjects with hypertrophic scars were enrolled, out of which 27 subjects had burn scars, 15 subjects had post-traumatic and post-operative scars, 7 subjects had acne scars and 6 subjects had other scars. All the subjects were treated with ultra-pulsed CO2 fractional laser (with the mode of CW, Active FX, Dee pFX, SCAAR FX) , and were asked to apply grinded "meibao" bum moist cream for 2 weeks. The total duration of the treatment period was 3 months, during which the subjects underwent 3 to 8 therapeutic sessions. Compound betamethasone combined with 5-Fu were injected to the scar. The pre-therapeutic and post therapeutic assessment of pain, pruritus, wound healing time, the incidence of pigmentation were done and each of the subjects were given a score by Vancouver scar rating scale. Results Pain and pruritus improved significantly, the incidence of pigmentation and the score by Vancouver Scar Scale were significantly less than those prior to treatment. The difference was statistically significant. Conclusion Ultra-pulsed CO2 fractional laser combined with drug injection therapy can effectively treat hypertrophic scar.
