摘　　要：目的 ：观察血管对比剂对毛细管血清蛋白电泳的影响,并探讨其解决方案。方法 ：1例64岁男性患者的毛细管血清蛋白电泳结果中α2区出现一明显异常峰,而利用透析方法处理后,该异常峰百分比明显降低。利用不同浓度的欧乃派克行血管对比剂对毛细管血清蛋白电泳的干扰试验,以验证欧乃派克在毛细管血清蛋白电泳中形成的异常峰,同时再以琼脂糖血清蛋白电泳进行验证。同时检测患者的血清蛋白及常规肿瘤标志物。结果：经透析后,患者标本中的α2区异常峰从83.6%降至32.8%。欧乃派克干扰试验结果表明,加入不同水平的欧乃派克后,毛细管血清蛋白电泳图谱中α2区的异常峰为45.7%~6.9%不等。静脉注射欧乃派克后采集的外周血,经透析处理前、后的标本及欧乃派克干扰试验的所有检测标本的琼脂糖血清蛋白电泳结果在α2区未见异常峰,免疫固定电泳呈阴性结果,提示患者及干扰试验中α2区异常峰为非蛋白组分。患者的常见肿瘤标志物检查结果均在参考区间内。结论：欧乃派克对比剂对毛细管血清蛋白电泳图谱结果存在较大影响,应采取琼脂糖电泳加以验证,并对使用血管对比剂患者采取适当措施和建立合理的采样流程,以避免其对试验结果的干扰。Objective To investigate the influence of contrast agent on serum capillary zone electrophoresis （CZE） and its resolution. Methods An abnormal apha2 peak in serum CZE of one 64-year old man was seen, but the result of immunofixation electrophoresis was negative. When the sample was dialyzed, the abnormal apha2 peak was reduced. For investigation, different concentrations of Omnipaque were added to a normal sample to verify the interference of contrast agent on the serum CZE, and agarose electrophoresis was used to detect if the abnormal peak is protein or not. Routine tumor markers of the sample were determined. Results The abnormal apha2 peak of the serum CZE decreased from 83.6% to 32.8% after dialysis. The serum agarose electrophoresis profile of the sample was normal. The percentage of apha2 peak of the normal sample added with different concentrations of Omnipaque were from 45.7% to 6.9%. The profiles of agarose electrophoresis of all of the samples were negative, which demonstrated that abnormal peak of serum CZE was not protein. The routine tumor markers were within the reference interval. Conclusions Some contrast agent, such as Omnipaque can interfere the serum CZE profiles. The clinical laboratory should pay attention to the patients who are injected with contrast agent just before blood collection.
