摘  要:目的观察光感基因调控技术对骶上脊髓损伤所致神经源性膀胱功能的影响。方法 50只大鼠经尿流动力学检查无异常后进行随机分组,并采用T10脊髓完全横断建立脊髓损伤动物模型,分为假手术对照组、脊髓损伤无蓝光刺激组和脊髓损伤蓝光刺激组,利用HE染色技术对骶髓行蓝光照射前后膀胱的形态学变化进行观察,并且通过Real time PCR技术检测毒蕈碱型乙酰胆碱受体(muscarinic acetylcholine receptor)M2、M3和β3肾上腺素能受体(β3-adrenergic receptor,β3-AR)在大鼠膀胱逼尿肌中的表达。结果与脊髓损伤无蓝光刺激组相比,在HE染色结果中脊髓损伤蓝光刺激组逼尿肌细胞形态大小相对均一,肌纤维排列较有序,趋近于假手术组。Real time PCR技术检测结果显示,与假手术组相比,M2-AR mRNA和M3-AR mRNA的表达在脊髓损伤无蓝光刺激组显著升高(P<0.05);而经蓝光照射一定时间后,上述两种受体mRNA的表达降低(P<0.05),β3-AR mRNA的表达与M-AR表达相反。结论光感基因技术可以通过调控逼尿肌受体修复骶上脊髓完全性损伤后神经源性膀胱。Objective To observe of the effect of light gene regulation on neurogenic bladder function recovery by the upper-sacral spinal cord injury in rats. Methods 50 rats which had no abnormalities after urine flow dynamic test were randomized and the 10 thoracic spinal cord complete transection animal model was set up,dividing into the control group,no blue light stimulation of spinal cord injury group and blue light stimulation of spinal cord injury group. The bladder morphological changes before and after the use of blue light stimulation were observed by HE staining technique. Furthermore,the express of M-AR and β-AR were tested by Real time PCR in the rat bladder detrusor. Results Compared with no blue light stimulation of spinal cord injury group,detrusor cells were relatively uniform shape and size while the more muscle fibers arranged in an orderly,closing to control group. Real time PCR technical results show that,compared with the control group,the expression of M2-AR mRNA and M3-AR mRNA in no blue light stimulation of spinal cord injury group was significantly higher(P〈0. 05) while the express was lower in the blue light stimulation of spinal cord injury group(P〈0. 05). β3-AR mRNA expression and M-AR expression were contrary. Conclusion Light gene regulation can repair the neurogenic bladder dysfunction of upper-sacral spinal cord injury by regulating the detrusor receptors.
