摘 要:目的分析口服75 g葡萄糖耐量试验(OGTT)三时点血糖值与孕妇妊娠结局的相关性。方法选取2015年1—12月于复旦大学附属上海市第五人民医院产科住院分娩的妊娠期糖尿病(GDM)患者125例、正常妊娠妇女157例为研究对象,分为GDM组和对照组,分析三时点血糖值与妊娠结局的相关性。结果 OGTT 1 h血糖值所确定的GDM与产后出血、新生儿窒息的发生率之间关联,列联系数分别为r=0.015,r=0.144,差异有统计学意义(P<0.01);OGTT 2 h血糖所确定的GDM与新生儿窒息的发生率之间关联,列联系数分别为r=0.143,差异有统计学意义(P<0.01)。结论在GDM患者中,OGTT 1 h、2 h血糖值与新生儿窒息存在相关性,OGTT 1 h血糖值与产后出血存在相关性,因此,对于这两时点异常的GDM患者需警惕新生儿窒息、产后出血的发生。Objective To investigate the correlation of plasma glucose levels in 3-time point 75 g oral glucose tolerancetest(OGTT)with the pregnancy outcome in the patients with gestational diabetes mellitus(GDM)after prenatal intervention.Methods A total of 125 GDM patients(GDM group)and 157 pregnant women(control group)with normal glucose,who allgiven birth in our department from January to December 2015 were enrolled in this study. Three-time point OGTT was carriedout,and the results were analyzed with the pregnancy outcome. Results The patients with GDM determined by the value of 1-hour post-load plasma glucose had higher probabilities of postpartum hemorrhage and neonatal asphyxia(r =0.015,r =0.144,P〈0.01). And if the GDM was identified by 2-hour post-load plasma glucose,it was also correlated with neonatal asphyxia(r =0.143,P〈0.01). Conclusion For the GDM patients,1- hour and 2- hour post- load plasma glucoses are associated withneonatal asphyxia,and the former value is also with postpartum hemorrhage. Thus,clinicians should alert to the occurrence ofneonatal asphyxia and postpartum hemorrhage when the GDM patients have abnormally 1-hour and 2-hour post-load plasmaglucoses.
