经皮椎体成形术与AF钉内固定治疗老年性椎体压缩性骨折的临床效果对比

摘　　要：目的：比较经皮椎体成形术（PVP）与AF钉内固定治疗老年性椎体压缩性骨折的临床效果。方法：选取2012年5月~2015年2月某院收治的90例老年椎体压缩性骨折患者作为研究对象,采用随机数字表法将所选患者分为研究组和对照组,每组各45例,研究组采用经皮椎体成形术,对照组采用AF钉内固定治疗,使用视觉模拟评分法（VAS）评价患者的疼痛程度,综合比较两组患者治疗总有效率、治疗前后VAS疼痛评分、椎体前缘高度、Cobb’s角及并发症情况的差异。结果：研究组治疗总有效率高于对照组的77.8%（P〈0.05）;治疗后研究组患者VAS疼痛评分低于对照组（P〈0.05）,椎体前缘高度、Cobb’s角均明显优于对照组（P〈0.05）;研究组患者出现1例骨水泥椎间盘渗漏,对照组出现2例椎体疼痛,两组并发症发生率比较差异无统计学意义（P〉0.05）。结论：经皮椎体成形术治疗老年性椎体压缩性骨折效果确切,操作简单,可明显改善骨折疼痛症状,促进患者早日康复,优于AF钉内固定治疗,值得临床推广使用。Objective：To compare clinical effect between Percutaneous vertebral plasty and AF nail internal fixation in the treatment of vertebral compression fractures for the elderly.Methods：90cases with vertebral compression fractures admitted in a hospital from May 2012 to February 2015 were taken as the research objects,who were divided into research group and the control group,with 45 cases in each group.The research group were treated with percutaneous vertebral plasty,and the control group were treated with AF nail internal fixation using visual analogue scale（VAS）to evaluate the patient＇s pain degree,then compare the total effective rate,VAS pain score,fanterior flange height,Cobb＇s Angle and complications of difference before and after treatment.Results：The total effective rate of the research group was higher than that of the control group 77.8%（P0.05）;after treatment,VAS pain score of the research group was lower than the control group（P0.05）,fanterior flange height,Cobb＇s Angle were significantly better than that of control group（P0.05）;Group patients 1case of bone cement leakage of intervertebral disc,the control group in 2cases of vertebral body pain,complications was similar between the two groups has no statistical significance（P0.05）.Conclusion：Percutaneous treatment of vertebral compression fractures in the elderly vertebral plasty effect is exact,with simple operation,which can obviously improve the fracture pain symptoms,promote patients recover at an early date,and is better than that of AF internal fixation treatment,worth clinical promotion.
