摘　　要：目的探讨夫妇双方的孕前因素与自然流产的相关性。方法采用前瞻性研究,对参加上海市2012年免费孕前优生健康检查的8个区县的6 319对夫妇进行随访,随访2012年参检后近3年内自然流产的发生情况,结合2012年免费孕前优生健康检查数据,采用多因素Logistic回归模型分析孕前因素与流产相关性。结果在调整混杂因素后,孕前女性乙型肝炎表面抗体为阴性者[调整后OR（a OR）1.9,95%CI 1.2~3.3）、不良妊娠结局史（a OR 2.5,95%CI 1.7~3.7）、自然流产史（a OR 2.7,95%CI 1.8~4.0）、糖尿病家族史（a OR 1.6,95%CI 1.1~2.5）及白带清洁度为Ⅲ度者（a OR 1.6,95%CI 1.0~2.7）发生自然流产的风险增加。男性乙型肝炎表面抗体阴性（a OR 2.2,95%CI 1.3~3.9）、接触重金属（a OR 3.3,95%CI 1.1~9.7）及男性人际关系紧张（a OR 2.3,95%CI 1.1~4.5）,则妻子发生自然流产的风险增加。结论夫妻双方孕前疾病史、有害物质接触等孕前因素增加自然流产的风险。Objective To probe the relation of female and male prepregnancy risk factors to miscarriage.Methods We followed up 6 319 couples who had a prepregnancy health examination in 2012 in 8 districts in Shanghai. A multivariable Logistic regression model was used to examine the association between prepregnancy risk factors and miscarriage. Results In women,hepatitis B surface antibody negative [adjusted odds ratio（ a OR） 1. 9,95% confidence interval（ CI） 1. 2- 3. 3 ],history of adverse pregnancy outcomes（ a OR 2. 5,95% CI 1. 7- 3. 7）,history of spontaneous abortion（ a OR 2. 7,95% CI 1. 8- 4. 0）,family history of diabetes（ a OR 1. 6,95% CI 1. 1- 2. 5）,and 3rd degree leucorrhea cleanliness（ a OR 1. 6,95% CI 1. 0- 2. 7） increased the risk of miscarriage. In men,hepatitis B surface antibody negative（ a OR 2. 2,95% CI 1. 3- 3. 9）,exposure to heavy metals（ a OR 3. 3,95% CI 1. 1- 9. 7）,and interpersonal tension（ a OR 2. 3,95% CI 1. 1-4. 5） increased the risk of miscarriage of their wives. Conclusion Couples＇ prepregnancy diseases history,contact with harmful substances and other prepregnancy risk factors increase the risk of spontaneous abortion.
