运用鱼骨图分析非计划性拔管的原因及对策

摘　　要：目的：运用鱼骨图分析非计划性拔管的原因及对策，降低非计划性拔管的发生率。方法：分析某三级甲等综合医院2014年1~12月25例非计划性拔管事件的原因，绘制鱼骨图。结果：鱼骨图分析显示非计划性拔管的原因有人、物、法、环4个方面，降低非计划性拔管发生率的对策有改进管理、强化培训、重视宣教。结论：运用鱼骨图可以找到非计划性拔管的根本原因，制定有效措施。Objective：To analyze the reasons and countermeasures of unplanned extubation with fishbone diagram and lowerits incidence. Methods：Ana-lyzed reasons of the 25 unplanned extubation events happened in a third - grade class - A general hospital from January to December in 2014,and drew a fishbone diagram. Results：The fishbone diagram analysis showed that there were four aspects in reasons of unplanned extubation events,including people, object,method and environment,and that there were such countermeasures for reducing incidence of unplanned extubation as improving management, strengthening training and paying attention to propaganda and education. Conclusion：Using fishbone diagram can find out the root causes of unplanned ex-tubation and formulate effective measures.
