摘  要:目的探讨正常儿童左心室局部心肌收缩期周向峰值应变(SC)及应变率(SrC)与年龄、心率的关系。方法收集164名正常儿童胸骨旁左心室短轴基底段、中间段及心尖水平二维动态图像,采用GE EchoPAC工作站分析,并获得左心室18节段SC及SrC值,将其与年龄和心率进行回归分析。结果儿童18个节段的周向应变及应变率曲线形态基本一致。基底段、中间段和心尖段的前间隔、后间隔、下壁SC较高,侧壁与后壁较低。基底段、中间段和心尖段不同部位SrC差异有统计学意义(P均<0.05)。除基底段和中间段的前间隔、前壁、后间隔和心尖段的后间隔以外,其余节段与年龄存在相关性;除前间隔所有节段、前壁中间段及心尖段、后间隔心尖段外,其余节段SC与心率有相关性;而中间段下壁SC与心率和年龄均有相关性(R^2=0.169,P=0.032)。基底段和中间段的后壁及下壁、中间段的侧壁、心尖段的前间隔、前壁、侧壁、后壁、下壁和后间隔SrC与年龄无相关性,其余节段与年龄存在相关性。基底段的后壁、心尖段的前壁、侧壁、后壁SrC与心率无相关性,其余节段SrC均与心率有相关性(P均<0.05)。心尖段的前间隔SrC与心率和年龄均有相关性(R^2=0.053,P=0.044)。结论正常儿童局部心肌SC及SrC存在节段间的差异,且与心率和(或)年龄存在相关性。采用斑点追踪成像技术评价儿童左心室局部心肌SC及SrC时需考虑年龄、心率因素。Objective To evaluate the relationship of circumferential systolic peak strain (SC) and circumferential systolic peak strain rate (SrC) with age and heart rate in healthy children with speckle tracking imaging. Methods Totally 164 normal children were enrolled. The dynamic 2-dimensional gray images of left ventricular short axis views at the basal lev- el, middle level and apex were obtained. All images were analyzed off-line in GE EchoPAC workstation and SC, SrC of left ventricular 18 segments were acquired, and then regression analysis between SC or SrC and age, heart rate were per- formed. Results The shape of the SC curves and the SrC curves of the left ventricular regional myocardium were similar. Regional SC in the basal segments, middle segments and apical segments of anterior septum, posterior septum and inferior wall were high, and the lateral wall and posterior wall were low. There were significant differences in SrC in the basal, middle and apical segments (all P〈0.05). Regional SC correlated with age except the basal and middle segments of anteri- or septum, anterior wall, posterior septum and apical segment of posterior septum, and correlation was found with heart rate in segments except anterior septum of three segments, the middle and apical segments of anterior wall, and the apical segment of posterior septum; SC in the inferior wall of middle segment was influenced both by age and heart rate (R2= 0. 169, P=0. 032). Correlation was found with age in segments for regional SrC except basal and middle segments of pos- terior wall, middle segment of lateral wall, apical segments of anterior septum, anterior wall, lateral wall, inferior and psterior wall (all P〈0.05). Regional SrC correlated with heart rate except basal segment of posterior wall, apical seg- ments of anterior, lateral and posterior wall.
