椎弓根钉内固定联合伤椎植骨治疗胸腰骨折

摘　　要：目的:探讨椎弓根钉内固定联合伤椎植骨治疗胸腰椎骨折的临床效果。方法:选取2010年11月-2015年9月本院收治的55例胸腰椎骨折患者作为研究对象,根据治疗方式不同分为对照组19例和观察组36例。对照组采用椎弓根钉内固定治疗,观察组采用椎弓根钉内固定联合伤椎植骨治疗,比较两组治疗前后和术后1年随访椎体前缘高度、Cobb角变化情况及并发症总发生率。结果:两组患者治疗前前缘高度和Cobb角比较差异均无统计学意义(P>0.05);治疗后,两组前缘高度和Cobb角均较治疗前改善,比较差异均有统计学意义(P<0.05),但两组间比较差异无统计学意义(P>0.05)。术后1年对照组前缘高度丢失程度和Cobb角丢失度明显高于观察组,比较差异均有统计学意义(P<0.05)。观察组并发症发生率为5.56%,明显低于对照组的36.84%,比较差异有统计学意义(P<0.05)。结论:椎弓根钉内固定联合伤椎植骨治疗胸腰椎骨折的临床效果显著,具有创伤小、安全可靠的优势,可有效将患者体内骨缺损空腔填充,恢复脊柱的稳定性,减少椎体高度的丢失,提高患者生活质量,具有较高临床应用价值。Objective： To explore clinical effects of internal pediele screw fixation combined with vertebra bone graft for treatment of thoracolumbar bone graft fractures.Method： From November 2010 to September 2015, 55 cases of thoracic and lumbar fractures treated in our hospital were selected as the research objects, they were divided into the control group of 19 cases and observation group of 36 cases according to the different treatment methods.The control group was treated with pedicle screw fixation and the observation group was treated with pedicle screw fixation combined with bone graft.The anterior height of vertebral body, Cobb angle change and incidence of complications of two groups before and after treatment and followed up after 1 years of operation were compared. Result： There were no significant differences in anterior height of vertebral body and Cobb angle change of two groups before treatment （ P〈0,05 ） ; after treatment, anterior height of vertebral body and Cobb angle change of two groups were improved than before treatment, the differences were statistically significant （ P〈0.05 ）, but there was no significant difference of two groups （ P〉0.05 ） . Leading edge height loss and Cobb angle loss in the control group were higher than observation group 1 years after operation, the differences were statistically significant （ P〈0.05 ） . The complication rate of the observation group was 5.56%, which was significantly lower than 36.84% of the control group, the difference was statistically significant （ P〈0.05 ） .Conclusion： Internal vertebral pedicle screw fixation in combination with vertebral bone graft delivers significant effect in the treatment of thoracolumbar fractures, this technique provide advantages of small-size trauma, safety and reliability, it can effectively fill cavities and bone defects in vivo, restore the stability of the spine, reduce the loss of vertebral height and improve life quality of patients, it has high clinical promotion value.
