摘  要:目的了解慢性肾脏病(CKD)患者血液透析前后肾纤维化指标及肾功能的变化情况。方法收集2009年9月-2013年8月本医院收治的150例CKD患者,同时选择同期体检健康者40例为对照组。观察CKD患者血液透析前后肾纤维化指标血清透明质酸(HA)、血浆纤维连接蛋白(FN)、层粘蛋白(LN)、血浆Ⅲ型前胶原(PC-Ⅲ)和Ⅳ型胶原(C-Ⅳ)及肾功能指标血尿素氮(BUN)、血肌酐(Scr)和白蛋白(ALB)的变化情况。结果与血液透析前相比,CKD患者透析后HA、LN、PC-Ⅲ和C-Ⅳ水平及BUN、Scr和ALB水平均明显下降,而FN和Ccr水平明显升高,差异均具有统计学意义(P<0.05)。但无论是透析前还是透析后,CKD患者HA、LN、PC-Ⅲ和C-Ⅳ及BUN、Scr和ALB水平与健康对照组相比均显著增高,而FN和Ccr水平呈显著下降(P<0.05)。肾功能分级疗效结果显示,血液透析对改善早中期CKD患者肾功能损伤具有一定疗效。结论 CKD患者经血液透析后可改善肾纤维化指标及肾功能,从而缓解肾损伤状况。Objective To explore the changes of the levels of renal fibrosis and renal function of patients in chronic kidney disease(CKD)before and after hemodialysis.Methods 150 patients with CKD collected from my hospital were enrolled in the study from September 2009 to August 2013.40 health control were considered as control group.The changes of the level of serum hyaluronic acid(HA),plasma fibronectin(FN),laminin(LN),pro-collagen typeⅢ(PC-Ⅲ)and collagen typeⅣ(C-Ⅳ),and blood urea nitrogen(BUN),serum creatinine(Scr)and albumin(ALB)in CKD were analyzed.Results Compared with before hemodialysis,the levels of HA,LN,PC-Ⅲ and C-Ⅳ,and BUN,Scr and ALB in CKD patients after hemodialysis were significantly decreased,but increased in FN and Ccr(P〈0.05).However,compared with the control gourp,the levels of HA,LN,PC-Ⅲ and C-Ⅳ,and BUN,Scr and ALB in either before hemodialysis or after hemodialysis were significantly increased,but decreased in FN and Ccr(P〈0.05).Clinical grading of renal function showed that hemodialysis had positive effect on renal function impairment at the early-middle stage of CKD patients.Conclusion Hemodialysis treatment can improved Renal fibrosis and renal function of patients in CKD,thus relieve the status of kidney injury.
