输尿管镜下钬激光碎石术与切开取石术治疗输尿管结石对照研究

摘　　要：目的比较输尿管镜下钬激光碎石术与切开取石术治疗输尿管结石的临床疗效。方法选取2010年1月-2015年6月在我院择期行外科手术治疗的输尿管结石患者157例,根据手术方式分为钬激光组（n=110）和常规组（n=47）,分别为输尿管镜下钬激光碎石术与输尿管切开取石术。比较两组手术时间、输尿管结石清除情况、术后疼痛、并发症发生情况及住院时间。结果两组患者输尿管结石总清除率、输尿管上段结石清除率和输尿管中下段结石清除率比较,差异无统计学意义（P〉0.05）;钬激光组手术时间、术后住院时间均明显短于常规组（P〈0.05）;钬激光组术后VAS评分明显低于常规组（P〈0.05）;两组患者术后并发症发生率比较,差异无统计学意义（P〉0.05）。结论输尿管镜下钬激光碎石术与切开取石术相比,同样有效清除输尿管结石,而缩短手术时间和住院时间,疗效满意。Objective To compare the clinical efficacy of holmium laser lithotripsy under ureter mirror and ureterotomy treatment for ureteral calculi. Methods 157 patients with ureteral calculi who scheduled for elective surgical treatment between January 2010 to June 2015 were selected, and divided into holmium laser group（n=110） and conventional group（n=47） according to the operation mode. The patients in two groups were given holmium laser lithotripsy under ureter mirror and ureterotomy respectively.The operative time,rate of ureteral stone clearance, postoperative pain,complication situation and hospital stay were compared between the two groups. Results There was no significantly difference between the patients in two groups on the rate of ureteral stone clearance upper ureteral caiculi middle lower ureteral caiculi（P〉0.05）.The operative time,postoperative hospital stay of holmium laser group were significantly lower than that of conventional group（P〈0.05）. The VAS score of holmium laser group was significantly lower than that of conventional group（P〈0.05）.There was no statistically significant difference between the two groups on the complication（P〉0.05）. Conclusion Holmium laser lithotripsy under ureter mirror and ureterotomy both can effectively upper ureteral calculi,but holmium laser lithotripsy under ureter mirror can more shorten the operative time and hospital stay,and have better security.
