摘  要:目的应用血管回声跟踪(ET)技术评价肥胖儿童颈总动脉弹性,探索儿童期肥胖对动脉弹性的影响。方法应用ET技术检测30例肥胖儿童双侧颈总动脉弹性参数:颈总动脉内中膜厚度(IMT)、压力应变弹性系数(Ep)、僵硬度(β)、顺应性(AC)和脉搏波传导速度(PWVβ),并与30例正常儿童对照组比较。结果肥胖组IMT、PWVβ、AC分别为(0.44±0.09)mm、(5.22±1.06)m/s、(1.79±0.28)mm2/k Pa,对照组IMT、PWVβ、AC分别为(0.42±0.10)mm、(6.88±1.04)m/s、(2.50±0.54)mm2/k Pa,2组比较差别无统计学意义(t=0.77、1.52、1.69,均P>0.05),肥胖组β、Ep分别为(3.99±1.89)、(56.84±17.94)k Pa,对照组β、Ep分别为(3.11±1.61)、(39.45±17.19)k Pa,肥胖组较对照组增高,差别有统计学意义(t=3.41、5.13,P<0.05或P<0.01)。结论肥胖儿童颈总动脉弹性在内中膜出现形态学改变前已有下降。Objective To apply echo-tracking (ET) to evaluate cephalic artery elasticity of obese children, explore the effect of childhood obesity on cephalic artery elasticity. Methods ET was used to detect parameters of bilateral cephalic arteries of 30 obese children: intima media thickness (IMT), pressure-strain elasticity modulus (Ep), β-stiffness parameter (13), arterial compliance (AC), pulse wave velocity (PWVβ) , then the results were compared with 30 healthy children in control group. Results IMT, PWVβ, and AC in obesity group were (0.44±0.09) mm, (5.22±1.06) m/s, and ( 1.79±0. 28) mm2/kPa, respectively; IMT, PWVβ, and AC in control group were (0. 42±0. 10) mm, (6. 88±1.04) m/s, and (2. 50±0. 54) mm2/kPa, respectively, there was no statistically significant difference between the two groups (t=0. 77, 1.52, 1.69, P〉0. 05) . β and Ep in obesity group were (3.99±1.89) and (56. 84±17.94) kPa, respectively; 13 and Ep in control group were (3.11±1.61) and (39.45±17. 19) kPa, respectively, β and Ep in obesity group were statistically significantly higher than those in control group (t = 3.41, 5. 13, P〈0. 05 or P〈0. 01 ) . Conclusion Cephalic artery elasticity decreases before morphological change of intima media thickness among obese children.
