延香镇痛胶囊联合利多卡因碘油乳剂在经导管动脉内化疗栓塞中的止痛效果研究

摘　　要：目的观察延香镇痛胶囊联合利多卡因碘油乳剂在经导管动脉内化疗栓塞中的止痛效果。方法纳入行经导管动脉化疗栓塞术的85例患者随机双盲地分为观察组（n=42）和对照组（n=43）。对照组患者术前给予利多卡因碘油乳剂止痛,观察组给予延香镇痛胶囊联合利多卡因碘油乳剂止痛。对所有患者术中Wong-Baker脸谱疼痛评分、术后视觉模拟疼痛量表（VAS）评分、术后疼痛持续时间、术后焦虑状态（SAS）评分、脉率、术后疼痛级别进行评定,观察患者不良反应发生情况。结果观察组术中WongBaker脸谱疼痛评分、术后VAS评分、术后疼痛持续时间均优于对照组（P〈0.05）。观察组术后SAS评分、脉率均优于对照组（P〈0.05）。观察组术后疼痛级别在轻度疼痛以下患者比例显著高于对照组（P〈0.05）。观察组不良反应比例与对照组比较差异无统计学意义（7.14%vs.13.95%,χ^2=1.041）。结论延香镇痛胶囊联合利多卡因碘油乳剂对实施经导管动脉内化疗栓塞术患者有良好的止痛效果,可减轻患者疼痛程度,改善患者焦虑状态,联合用药优于单独用药。Objective To observe the analgesic effect of Yanxiang analgesic capsules combined with hdocaine hpiodol emulsion in transcatheter arterial chemoembolization. Methods 85 patients were randomly divided into double-blind study group （ n = 42） and control group （ n = 43 ）. Control group before lipiodol emulsions received lidocaine, and observation group received Yanxiang analgesic capsule combined with lidocaine-lipiodol emulsions. The intranperative Wong-Baker mask pain scores, postoperative VAS score, duration of postoperative pain, postoperative anxiety （SAS score）, pulse rate, postoperative pain level were assessed and adverse events were observed in patients. Results The intranperative Wong-Baker mask pain scores, postoperative VAS score, duration of postoperative pain in observation group were better than control group （P 〈 0. 05 ）. The postoperative anxiety （ SAS score）, pulse rate in observation group were better than control group （ P 〈 0. 05 ）. The pain rate of mild pain or below in observation group was higher than control group（7. 14% vs. 13.95% ,χ^2 = 1. 041 ）. Conclusion The Yanxiang analgesic capsules combined with lidocaine lipiodol emulsion has good analgesic effect on the patients with implementation of canon transcatheter arterial chemoembolization, could reduce the levels of pain, improve anxiety, and the combination therapy is superior to single drug.
