护理结局分类系统在机械通气患者口腔护理中的应用研究

摘　　要：目的探讨护理结局分类系统(Nursing Outcomes Classification,NOC)在机械通气的患者口腔护理中的临床应用效果。方法选取我院重症监护室经口气管插管进行机械通气的患者87例,按照病历号顺序进行编号,应用随机数字表法分为观察组48例和对照组49例。对照组采用传统口腔护理流程,观察组根据"护理结局分类系统"中的口腔卫生评分标准评价患者口腔情况,根据评价结果指导护理细节操作,在患者插管第1天、第3天、第7天时分别采用"护理结局分类系统"中的口腔卫生评分标准进行评分,比较评分结果。结果两组患者在插管第1天、第3天、第7天时NOC-口腔卫生评分结果分别为(79.74±7.25)分、(79.89±7.96)分、(83.95±5.01)分、(90.05±3.55)分、(85.79±3.63)分和(100.33±1.46)分,插管第3天及插管第7天时两组比较差异有统计学意义(P<0.01)。结论护理结局分类系统能够很好地指导气管插管患者的口腔护理重点,充分发挥护理工作者、健康照顾者、教育者、咨询者、决策者、管理者的角色功能。Objective To Study the effect of nursing outcomes classification（NOC） in patients＇ oral care with mechanical ventilation, and to evaluate its clinical application effect. Methods 87 patients in our department with mechan- ical ventilation by oral trachea cannula was selected and randomly divided into two groups. By using the method of random number table, 48 cases as observation group and 49 cases as control group according to the medical record number order. Patients in control group adopts the traditional process of oral care, patients in observation group was cared based on the nursing outcomes classification of oral cavity, and based on the evaluation results to guide nursing operation details, using the nursing outcomes classification of oral hygiene rating criteria to assess the patients＂ situa- tion at 1 day, 3 day and 7 day respectively. Results The results of intubation NOC-oral health score between control group and experimental group patients at 1 day, 3 day and 7 days are 79.74± 7.25,79.89 ± 7.96, 83.95 ± 5.01,90. 05±3.55, 85.79±3.63 and100.33±1.46 respectively,and the 3 day and 7 days intubation for the two groups are statistically significant （P〈0.01）. Conclusions Nursing outcomes classification can be a good guide oral nursing care for patients with mechanical ventilation, It＇s given full play to the care of health care workers, educators, consultants, policy makers and managers＂ role function.
