摘  要:目的:观察阿托伐他汀对急性冠脉综合征患者循环中可溶性fractalkine表达的影响。方法:选择经冠脉造影证实的急性冠脉综合征(ACS)患者40例,其中急性心肌梗死(AMI)20例(AMI组)、不稳定心绞痛(UAP)20例(UAP组)。另选择20例冠脉造影阴性的高脂血症患者作为对照组。3组均给予阿托伐他汀(20mg/次,1次/d)治疗3个月。用ELISA法检测治疗前后肘静脉血fractalkine水平;流式细胞技术检测肘静脉血CD14+单核细胞(PBMC)中CX3CR1表达。结果:入院时,AMI组[(1 047.0±348.9)pg/mL]和UAP组[(777.2±324.4)pg/mL]循环中fractalkine水平显著高于对照组[(485.1±260.9)pg/mL,P<20.001、P=0.004)];AMI组(38.8±15.4)%和UAP组(29.2±11.9)%循环中CD14+单核细胞CX3CR1表达率显著高于对照组[(15.2±7.9)%,P=0.009、P=0.03]。阿托伐他汀治疗3个月后,AMI组[(740.1±269.9)pg/mL]和UAP组[(523.8±188.5)pg/mL]fractalkine水平较治疗前显著下降(P=0.005、P=0.01)。结论:阿托伐他汀可明显降低急性冠脉综合征患者循环中fractalkine的表达,进而发挥抗动脉粥样硬化炎症免疫效应。Objective:To observe the effect of atorvastatin on the expression of soluble fractalkine in the circulation of patients with acute coronary syndrome.Methods:40patients with acute coronary syndrome(ACS)confirmed by coronary angiography were selected,including 20 patients with acute myocardial infarction(AMI group)and20patients with unstable angina pectoris(UAP group).Another 20 hyperlipidemia patients with negative coronary angiography were selected as the control group.All three groups were given atorvastatin 20 mg per time,one time per day for 3months.Fractalkine levels in elbow vein blood were detected before and after treatment by ELISA method.CX3CR1 expression in peripheral blood mononuclear cells(PBMC)was examined by flow cytometry.Results:On admission,fractalkine levels in the circulation of AMI group([1047±348.9]pg/mL)and UAP Group([777.2±324.4]pg/mL)were significantly higher than that of the control group([485.1±26.09]pg/ mL,P〈0.001,P=0.004);CX3CR1expression rates of CD14+ monocytes in the circulation of AMI group([38.8±15.4]%)and UAP group([29.2±11.9]%)were significantly higher than that of the control group([15.2±7.9]%,P= 0.009,P= 0.03).After 3 months of atorvastatin treatment,AMI([740.1±269.9]pg/mL)and UAP([523.8±188.5]pg/mL)fractalkine levels were significantly lower than before treatment(P=0.005,P=0.01).Conclusions: Atorvastatin can significantly reduce fractalkine expression in the circulation of acute coronary syndrome patients,and then achievethe immune effect of anti-atherosclerosis inflammation.
