钩突水平段影像学新分型对功能性内镜鼻窦手术的指导价值

摘　　要：目的:探讨功能性内镜鼻窦手术(FESS)中钩突水平段影像学新分型的价值。方法:选取慢性鼻-鼻窦炎患者135例,采用影像学钩突水平段新分型进行术前评估,在FESS术中进行观察和比较,评价其对手术指导的意义。结果:135例(270侧)患者进行钩突分型:内下型157侧(58.15%)、内上型81侧(30.0%)、外下型31侧(11.48%)、外上型1侧(0.37%)。所有患者均能在内镜下使用探针完成钩突切除术及上颌窦自然开口的扩大开放,1例外上型患者出现眶纸样板损伤并发症。结论:钩突水平段影像学新分型方法对评估钩突水平段切除和上颌窦口开放的难度以及预防手术并发症有一定指导意义,值得临床推广。Objective：To explore the value of the new imaging typing of the uncinate horizontal segment in functional endoscopic sinus surgery.Methods： A total of 135 cases of chronic rhino-sinusitis（CRS）were selected,and the new genotyping method of imaging uncinate horizontal segment was adopted before surgery.It was observed and compared during the functional endoscopic sinus surgery,and its significance in guiding the surgery was evaluated.Results：Uncinate typing was performed in 135 patients（270sides）：157sides were the internal lower type（58.15%）,81 the internal upper type（30.0%）,31 the external lower type（19.75%）and 1 the external upper type（0.37%）.We were able to complete resection of uncinate by using the probe under the endoscope and expansion of the natural opening of the maxillary sinus in all patients.The patient of the external upper type had orbital complications of paperacea injury.Conclusions：This study typing method to assess the level of uncinate resection and open maxillary sinus difficulty and prevent complications have a certain significance,which is worthy of promotion.The new imaging typing method of the uncinate horizontal segment has a certain guiding significance in assessing the difficulty of uncinate horizontal segment resection,opening the maxillary sinus and preventing of complications.It is worth popularizing clinically.
