后腹腔镜与经皮肾镜治疗复杂性输尿管上段结石对比  

Comparison of Retroperitoneal Laparoscopic and Percutaneous Endoscopic Treatment in the Treatment of Complicated Upper Ureteral Calculi

摘　　要：目的对比分析经皮肾镜和后腹腔镜用于治疗复杂性输尿管上段结石的临床效果。方法选取我院收治的29例复杂性输尿管上段结石患者,分为对照组15例(经皮肾镜治疗)和观察组14例(后腹腔镜治疗)两组,比对分析两组患者的治疗效果。结果经治疗后两组患者的病情全部有所好转,但对照组一次性净石率比观察组低,差异有统计学意义(P<0.05);且对照组在平均住院时间、术中出血量以及平均手术时间上都不如观察组的治疗效果明显,有统计学意义(P<0.05)。结论经皮肾镜和后腹腔镜在用于治疗复杂性输尿管上段结石患者都有一定的临床作用,但后腹腔镜在提高一次性净石率、控制术中出血量、缩短病患平均住院及术中时间上显然要比经皮肾镜好很多。Objective To compare the clinical effect of percutaneous renal and retroperitoneal laparoscopic surgery in the treatment of complicated upper ureteral＆amp;nbsp;calculi. Methods Selected our hospital of 29 cases of complicated upper ureteral calculi disease patients,divided into 15 cases in the control group（the treatment of percutaneous nephrolithotomy）and observation group（n=14）（after laparoscopic treatment）in the two groups,comparison and analysis of two groups of disease patient treatment effect. Results After treatment two groups of patient＆#39;s condition had improved,but the control group and the one-time net stone rate than in the observation group low,（P ＆lt; 0.05）,the difference was statistical y significant,and the control group in the average length of operation time,bleeding volume and mean operation time as observation group treatment effect were obvious（P ＆lt; 0.05）statistical significance. Conclusion By percutaneous nephrolithotomy and after laparoscopic,used in the treatment of complicated upper ureteral calculi patients has certain clinical effect,but after laparoscopic in enhancing the disposable net stone rate,control of intraoperative bleeding volume,shorten the average patient hospitalization and operation time obviously much better than by percutaneous nephrolithotomy.
