摘  要:目的:探讨"活血散风"针刺法调节卒中后高血压患者血管内皮功能的机制。方法:采用随机对照研究设计,对卒中后高血压患者治疗组采用"活血散风"针刺法干预30次,对照组采用"醒脑开窍"针刺法加拜新同(硝苯地平控释片)治疗。通过测量血压、测定患者血清中一氧化氮(NO)、内皮素(ET-1)浓度、ET-1/NO比值并与对照组比较,分析该针法在调节血管内皮功能方面可能的降压机制。结果:2组干预前后相比,无论收缩压还是舒张压均有不同程度的下降,但对照组收缩压降低较舒张压明显(P<0.05),治疗组收缩压、舒张压均显著降低(P<0.05),且治疗组比对照组能更好的降低卒中患者的舒张压;2组均可降低ET-1浓度、提高NO浓度、降低ET-1/NO比值(P<0.05);但与对照组干预后相比,治疗组干预后结果优于对照组(P<0.05)。结论:调节ET-1、NO浓度及ET-1/NO比值是活血散风针刺法的降压机制之一。Objective: To explore the mechanism of "blood-activating and wind-dispelling"needling method regulating vascular endothelial function of hypertension patients after stroke. Methods:By adopting randomized controlled study, the treatment group accepted the intervention of "blood-activating and wind-dispelling"needling method for 30 times,the control gronp adopted "restoring consciousness and hsuscitatior"needling methed and nifedipine extended-release the mechanism of reducing blood pressure of the needling method in regulating vascular endothelial function was analyzed by measuring blood pressure, detecting the concentrations of ET-1 and NO,calculating the ratio of ET-1/NO and comparing with the control group. Results: After the intervention, systolic pressure and diastolic pressure decreased to different degrees, but systolic pressure in the control group decreased more obvious compared with the decrease of diastolic pressure(P〈0.05), the decreases of systolic pressure and diastolic pressure in the treatment group were significant(P〈0.05), and diastolic pressure decreased better when the treatment group was compared with the control group; the concentrations of ET-1 decreased, the concentrations of NO raised but the ratio of ET-1/NO decreased in both groups(P〈0.05);the results of the treatment group were superior to these of the control group after the intervention(P〈0.05). Conclusion: Regulating the concentrations of ET-1 and NO, the ratio of ET-1/NO are one of the mechanisms of "blood-activating and wind-dispelling"needling method reducing blood pressure.
