透明质酸修护生物膜联合得宝松注射对兔耳增生性瘢痕的影响

摘　　要：目的观察透明质酸修护生物膜联合得宝松注射对兔耳早期增生性瘢痕的影响。方法新西兰白兔15只，建立兔耳增生性瘢痕模型。对照组：不予任何治疗；得宝松注射组（以下简称注射组）：局部注射得宝松0．1ml；透明质酸生物膜联合得宝松注射组（以下简称联合组）：局部注射得宝松0．1ml，同时每日2次外用透明质酸修护生物膜，疗程3周，行苏木素一伊红（HE）染色和CD34免疫组织化学染色观察及分析。结果对照组瘢痕色红，质硬；注射组和联合组瘢痕渐变平，色淡，质软。HE染色和CD34免疫组织化学染色，瘢痕增生指数（H1）、成纤维细胞密度（NA）、微血管密度（MVC）：对照组分别为3．07±0．24、5605．56±242．70、44．34±2．99；注射组分别为2．69±0．19、4393．00±245．03、37．42±2．87；联合组分别为1．82±0．26、3433．03±222．50、29．61±2．04。各组HI、NA、MVC两两比较差异均有统计学意义（P〈0．01），联合组对瘢痕的抑制作用优于注射组（P〈0．01）。结论透明质酸修护生物膜联合得宝松注射治疗兔耳增生性瘢痕，具有协同作用。Objective To observe the effect of combined application of hyaluronic acid （HA） re- pairing biological membrane and injection diprospan on hyperplastie sears in rabbit ears. Methods The ears of 15 rabbits were used to establish a model of hyperplastie sear according to previously reported meth- ods. Then, the rabbit ears were randomly divided into three groups： Control group receiving no treatment, injection group treated with diprospan Local injection 0. 1 ml） , combined group received diprospan Local injection （0. 1 ml） and treated with Hyaluronic acid repairing biological membrane （bid, for 3 weeks） top- ically, all the （ scar tissues were resected and subjected to pathological examination, statistical analysis was carried out by analysis of variance and rank sum test. Results The scars were deeply colored, thickened in the control group, but lightly colored, thinned and soft in the other two groups gradually. HE and CD34 immunohistochemistry staining showed the scar hyperplasia index （HI）, the fibroblast density （NA） and microvessel count （MVC） were 3.07 ＋ 0. 24, 5 605.56 ± 242. 70 and 44. 34 ± 2. 99 in the control group respectively, they were 2. 69 ± 0. 19, 4 393.00 ± 245.03 and 37.42 ± 2. 87 in the injection group and they were 1.82 -＋0. 26, 3 433.03 ±222. 50, 29. 61 ±2.04 the combined group respectively, As far as HI, NA and MVC were concerned, the group was differed significantly from the other two group in Muhiple compar- ison （ all P 〈 0.01 ）. And the inhibitory effect of the combined group on the hyperplastic scars was superior to that of the injection group （P 〈 0. 01 ）. Conclusion The combined application of Hyaluronie acid re- pairing biological membrane and injection diprospan can inhibit scar proliferation in rabbit ears synergisti- cally.
