摘  要:目的研究某驻军医院患者肺炎克雷伯菌的感染状况及耐药性,为医院感染控制及临床合理用药提供依据。方法通过细菌分离鉴定技术和药敏试验方法,对某驻军医院临床送检标本的检测结果进行分析。结果该驻军医院2013年度从患者送检标本中共分离出病原菌2 522株,其中肺炎克雷伯菌401株,占15.90%,主要分离自呼吸道标本和术后切口分泌物,其感染患者相对集中于老年病科、呼吸内科、重症监护病房及心血管内科。临床分离的肺炎克雷伯菌对氨苄西林、头孢唑啉、复方新诺明的耐药率较高,对喹诺酮类抗菌药物耐药率低于30%,对亚胺培南敏感。结论该医院肺炎克雷伯菌分离率较高,以呼吸道和手术感染切口为主,对临床常用抗菌药物产生不同程度的耐药。Objective To investigate Klebsiella pneumoniae infection and drug resistance in patients of a certain hospital during 2013,so as to provide medical basis for infection control and clinical medications. Methods Identification of pathogenic bacteria from clinic sample of patients was done through germ analysis system of type VITEK- 2 Compact,and drug susceptibility card of GN 13 and ATB G- 5 were used to conduct susceptibility test. Results 401 strains of Klebsiella pneumoniae were extracted during the whole year,which accounted for 15. 9% of pathogenic bacteria. Most of Klebsiella pneumoniae strains in infections were isolated from lower respiratory tract and wounds after surgical operation. Inpatient Area generally occurred in Gerontology Department,Respiratory Medicine Department,ICU and Vasculocardiology Department.Susceptibility test showed that Klebsiella pneumoniae drug resistance towards Imipenem was the lowest,which was 1. 2% and its drug resistance towards Amikacin,Piperacillin / Tazobactam,Levofloxacin,Ciprofloxacin,Amoxicillin / Clavulanic acid and Cefoxitin was lower that 30. 0%,while its drug resistance towards Ampicillin,Cefazolin,and Trimethoprim was higher. Conclusion Klebsiella pneumoniae strains show different drug resistance towards normal clinical antibacterial drugs and some have high drug resistance. The hospital should keep increasing detection rate of clinical samples and reinforce supervision of susceptibility test in order to facilitate clinical application of antibacterial drugs.
