摘  要:目的与常规扫描相比,评价超低剂量对比剂主动脉CT成像中联合应用能谱单能量成像和自适应统计迭代重建技术(ASIR)对于血管强化程度及图像质量的影响。方法收集2014年6月至2015年4月因怀疑主动脉夹层于我院行能谱CT扫描的62例患者,按先后顺序依照完全随机设计法编号,根据随机表随机分为实验组和对照组:实验组(31例)为超低剂量对比剂能谱扫描组,对比剂用量为0.4 m L/kg,注射速率为对比剂总量/(曝光时间+延迟时间),重建图像A组(55 ke V+50%ASIR组)和B组(70 ke V+FBP组);对照组(31例)为常规剂量对比剂120 k Vp扫描组,对比剂用量为70 m L,注射流率为5 m L/s,重建图像C组(120 KVp+FBP组)。测量三组主动脉及其各分支血管CT值,计算其对比噪声比(CNR),并分别记录实验组和对照组对比剂剂量及注射流率,采用单因素方差分析联合Bonferroni法进行差异性比较。采用Kruskal-Wallis检验三组图像质量主观评分进行比较分析。结果 A、B、C组主动脉各分支CNR:升主动脉CNR分别为(18.1±5.8)、(12.1±4.0)和(13.2±1.8),降主动脉CNR分别为(18.3±5.8)、(12.2±3.8)和(12.3±1.8),腹腔干CNR分别为(16.8±4.7)、(11.5±3.7)和(12.4±2.1),肾动脉CNR分别为(17.2±4.7)、(11.6±3.2)和(12.5±1.9),差异均有统计学意义(P值均<0.05);A、B、C组主动脉各分支CT值:升主动脉CT值分别为(358±69)、(213±91)和(374±34)HU,降主动脉CT值分别为(361±64)、(216±37)和(353±30)HU,腹腔干CT值分别为(336±55)、(205±34)和(354±36)HU,肾动脉CT值分别为(333±54)、(201±34)和(356±54)HU,两两比较A组与B组,B组与C组差异有统计学意义(P值均<0.05),A组与C组差异无统计学意义(P>0.05)。对照组C组每例患者对比剂用量为70m L,实验组每例患者人均对比剂用量为(28.6±4.5)m L,比较差异有统计学意义(P=0.000)。对照组注射流率为5.0 m L/s,实验组注射流率为(2.4±0.3)m L/s,比较差异有统计学意义(P=0.000)。结论能谱Objective To evaluate the vascular enhancement and image quality of ultra-low volume contrast medium at aortic CT angiography by using monochromatic images of spectral CT with ASIR reconstruction, in compared with conventional 120 kvp scan with 70 m L contrast medium. Methods Altogether 62 patients who underwent aorta CTA examination on a spectral CT scanner were evenly and random Ly divided into 2 groups: Experiment Group and Control Group. Experiment Group was scanned by using GSI mode with contrast agent volume of 0.4 m L/kg and injection rate calculated as volume /(delay time + exposure time). After the examination, images using 55 ke V(with 50%ASIR) and 70 ke V(with filtered back projection, FBP) were reconstructed. Those two sets of images were regarded as Group A and B. Control Group was scanned by using 120 k Vp with contrast agent volume of 70 m L and 5 m L/s injection rate. The obtained images using filtered back projection(FBP) were regarded as Group C. CT values and CNR of aorta and its branches were obtained and compared. Results were statistically analyzed by single-factor analysis of variance and Bonferroni correction among three groups. The subjective rating score among the three groups was compared by using Kruskal-Wallis H test. Results CNR values obtained in Group A, B and C were(18.1±5.8) vs(12.1±4.0) vs(13.2±1.8) in ascending aorta;(18.3±5.8) vs(12.1±3.8) vs(12.3±1.8) in descending aorta;(16.8±4.4) vs(11.5±3.7) vs(12.4±2.1) in celiac trunk;(17.2±4.7) vs(11.6±3.2) vs(12.5±1.9) in renal arteries, with significant differences among the three groups(P values0.05). CT values for aorta and its branches were [(358±69),(213±91) and(374±34) HU in ascending aorta;(361±64),(216±37) and(353±30) HU in descending aorta;(336±55),(205±34) and(354±36) HU in celiac trunk;(333±54),(201±34) and(356±54) HU in renal arteries for Group A, B and C respectively, with signific
