中医药内外三联法治疗膝关节骨性关节炎的临床疗效

摘　　要：目的：探讨中医药内外三联法治疗膝关节骨性关节炎的临床疗效和安全性。方法将2015年1~12月该院收治的90例膝关节骨性关节炎患者随机分为治疗组和对照组，各45例。治疗组给予内外三联法（骨九方汤剂内服、温经洗外用颗粒熏洗、玉龙散外贴）治疗，对照组采用西医常规治疗（口服盐酸氨基葡萄糖胶囊和塞来昔布胶囊），疗程均为8周。观察比较两组患者WOMAC骨性关节炎指数评分、中医证候积分的变化和不良反应发生情况。结果两组患者治疗后WOMAC骨性关节炎指数评分、中医证候积分均较治疗前明显下降，且治疗组下降程度显著优于对照组，差异均有统计学意义（P＜0.05或0.01）；治疗组治疗总有效率为93.33%（42/45），高于对照组的77.78%（35/45），差异有统计学意义（P＜0.05）。治疗组患者症状复发的阳性率为2.27%（1/44），低于对照组的17.07%（7/41），差异有统计学意义（P＜0.05）；治疗组患者不良反应发生率为8.8%（4/45），对照组为20.00%（9/45），两组比较，差异无统计学意义（P〉0.05）。结论中医药内外三联法治疗膝关节骨性关节炎疗效确切，安全性、稳定性好，值得在临床推广应用。Objective To investigate the safety and effect of the internal and external triple therapy of traditional Chinese medicine （TCM） in treating knee joint osteoarthritis. Methods Ninety cases of knee joint osteoarthritis in our hospital from Jan-uary to December were randomly divided into the treatment group and control group ,45 cases in each group. The treatment group was given the internal and external triple therapy（oral Gujiu Decoction,Wenjingxi External Application Granules for fuming and washing,Yulong Powders for external application）,while the control group adopted the Western medicine routine therapy（oral Glucosamine Hydrochloride Capsules and Celecoxib Capsules ）. The whole treatment course was 8 weeks. The WOMAC os-teoarthritis index,TCM symptom score and occurrence situation of adverse reactions were observed in the two groups. Results The WOMAC and TCM symptom scores after treatment in the two groups were significantly decreased compared with before treat-ment,moreover the decrease degree in the treatment group was significantly superior to that in the control group with statistic dif ference（P〈0.05 or 0.01）;the total effective rate in the treatment group was 93.33%（42/45）,which was higher than 77.78%（35/45） in the control group,the difference was statistically significant （P〈0.05）. The positive rate of symptom recurrence in the treat-ment group was 2.27%（1/44）,which was lower than 17.07%（7/41） in the control group,the difference was statistically（P〈0.05）;the adverse reaction rates were 8.8%（4/45） in the treatment group and 20.00%（9/45） in the control group with statistally significant dif-ference（P〈0.05）. Conclusion The internal and external triple TCM therapy in treating knee joint osteoarthritis has definite effect , good safety and stability,and is worthy of clinical promotion.
