血液灌流串联血液透析治疗急性有机磷中毒的疗效观察及护理对策

摘　　要：目的探讨血液灌流(hemoperfusion,HP)串联血液透析(hemodialysis,HD)在急性有机磷农药中毒(Organic phosphorus poisoning,简写AOPP)中的治疗效果及护理方法。方法选取北京老年医院2008年6月至2014年1月收治的急性有机磷农药中毒患者52例,分为对照组与观察组,每组各26例,对照组仅应用常规内科治疗,观察组在常规内科治疗基础上采用HD串联HP治疗,对比两组患者的治疗效果。结果观察组的观察指标昏迷时间、胆碱酯酶活性恢复时间及住院时间显著短于对照组[(2.1±1.1)h vs.(4.0±1.3)h;(4.2±1.1)h vs.(6.0±1.6)h;(7.0±1.4)d vs.(10.2±1.5)d],阿托品用量显著少于对照组[(490.7_+38.5)mg vs.(957.8±50.2)mg],P均<0.05。结论对于急性有机磷农药中毒患者采取HD串联HP治疗具有显著效果,在治疗中全面的护理干预可促使治疗效果进一步提升,促使治愈率提高,值得在临床中推广。Objective To investigate the clinical outcome and nursing strategy of hemodialysis（HD）combined with hemoperfusion（HP） in the treatment of acute organic phosphorus poisoning（AOPP）. Methods Fifty patients with AOPP were randomly divided into two groups： HD ＋ HP group（n=26）, HD ＋ HP in addition to the routine regimen, and the control group（n=26）, received routine regimen only. The effects of treatment were compared between these two groups. Results In terms of the time for the patients to regain consciousness, the time for cholinesterase activity to recover, the hospitalization time, and the usage of atropine, the HD ＋ HP group showed significantly lower values than the control group. Conclusion HD combined with HP is more effective than the routine regimen in the treatment of AOPP. Comprehensive nursing further improve the cure rate of AOPP.
